- KDA_3_10111 - --In the WIC, made on site roasted red pepper dip that contained sour cream, ranch dressing that contained buttermilk, and the alfredo sauce that contained heavy whipping cream did not have a prepared or discard date. PIC did not know when they were prepared. COS Food items were discarded.
- KDA_4_20211A2 - --Food contact plastic containers and lids were in the warewashing area, had cracks. COS Containers were removed and will be used a storage bins.
- KDA_4_60111A - --Stacked and stored as clean on a storage rack by the window, had stainless steel metal pans that had food debris on the exterior surface. COS Pan were removed to be washed, rinsed, and sanitized. A sliced that was stored on the a prep table had dried food debris on the food contact face plate. COS Slicer was removed to be washed, rinsed, and sanitized. In the warewashing area, food contact plastic bins had stickers and stickers residue. COS Bins were removed to be scrapped, washed, rinsed, and sanitized.
- KDA_4_60211E4 - --Suspected pink mold on the white dispenser in the ice mold. PIC said the ice machine will be returned to company.
- KDA_4_90311B - --Stainless steel pans were stored as clean were stacked wet. COS Pans were removed and placed in the warewashing to be washed rinsed, and sanitized.
- KDA_5_20515B - --The pipe underneath 3 compartment sink in the kitchen area, overflows with water on the floor when a large amount of water is drained.
- KDA_6_20213B1 - --The electrical insect zapper is hanging above clean stainless steel pans.
- KDA_6_30112 - --The hanswashing sink near the warewashing area did not have paper towels. COS Paper towels were provided during inspection.
- KDA_6_501111A - --In the back storage near the warewashing area had 40 to 50 drain flies on food contact equipment. FE has a contracted pest control provider. Could not provide a invoice.

Chlorine test strips were available. --In the WIC, multiple food items were had a date of when the food was prepared and frozen. The PIC sais the food was taken out of the freezer 10/27 at 3pm to thaw. Educated PIC and owner. --The establishment is in process of down sizing and rearranging the facility.
