  • KDA_3_50118A2 - --Located in the small RIC there was one quart of open milk with no open date. PIC said it was open on October 24. COS. Milk was dated.

  • KDA_Defacto - --Floors have been deep cleaned with power washer. All other items have been cleaned. --Light covers have been ordered. --Make table: cut lettuce 40.2F

  • KDA_3_50116A2 - --At 1:30pm, Located in the make table, there were food items being held above 41F. Cut lettuce 44.3F
  • KDA_3_50118A3 - --Located in the WIC, there was one container of hot dogs with a date of October 5. PIC said that was the day they were opened and placed in container. COS. Hot dogs were VD.
  • KDA_4_50111A - --The WIF has a large accumulation of ice and frost all over the unit. Frost is thick and resembles snow. PIC said it was because the door was not fitting properly. Door was fixed this morning.
  • KDA_4_60111A - --There was one ladle with dried food debris on the food contact surface. COS. Item was placed in the dish sink. Located at the shelving for clean pans, there were 15 food pans with dried food debris on the handles of the pans. COS. Pans were placed at dish area.
  • KDA_4_60111C - --The exterior of the soda machines have an accumulation of sticky debris and corrosion which cannot be cleaned off. PIC will call Soda company for replacement. The cabinets under the soda machines have a large accumulation of sticky debris and trash. The floor fan has a large accumulation of lent and grease. The ladder has a large accumulation of black sticky debris. The WIC and WIF doors and door handles have a large accumulation of black sticky debris. The RIC used for glass mugs has an accumulation of dried food debris on the shelving.
  • KDA_5_20515B - --Both hand sinks in the kitchen have constant leaks form the pipes under the basin. The 3vat sink has a constant drip from the faucet while it is turned on.
  • KDA_6_20211A - --Located in the kitchen area above the shelving for single use items, there are two light fixtures exposed and do not have covers or shields. PIC said they were broken and the new plastic needs cut to fit.
  • KDA_6_30111 - --The hand sink by the cashiers did not have any hand cleaning supplies. PIC noticed soap dispenser was not working correctly and possibly needed new batteries.
  • KDA_6_30211 - --The ladies toilet room did not have toilet paper in the larger stall. Both toilet rooms trash cans were overflowing with trash onto the floor. NOTE: both toilet rooms have an extremely strong sewer smell coming from the floor drains. PIC said a plumber is supposed to look at it.
  • KDA_6_50111 - --There is chipping paint and mold in the doorway and window frames in the dining area. The ceiling tiles in the kitchen have an accumulation of grease and dried food debris.
  • KDA_6_501111A - --There are approximately 15 live flies flying around the dining area and kitchen. Pest control company comes once a month.
  • KDA_6_50112A - --The ceilings in the dining area have an accumulation of dust debris. The closet by the toilet rooms has an accumulation of black debris on the floor. The back door in the kitchen has a large amount of black sticky debris. The floors in the dining area, under the soda machines, in the office area, and in the entire kitchen area have a large accumulation of grease and black sticky debris.
  • KDA_6_50112B - --The floor area by the hot water tank has an accumulation of aged dried rodent droppings.
  • KDA_6_50116 - --There was a wet mop laying scrunched inside the bucket and not hanging to air dry.
  • KDA_7_20111B - --Located on the preparation table, there was one bottle of glass cleaner sitting on the shelf above RTE ketchup and mustard packets. COS. Cleaner was moved.
  • KDA_7_20212A1 - --Located in the closet by the toilet rooms, there was one can of wasp/hornet cleaner. EPA #9688-233. None of the employees know what is was used for or why it was there. All employees are new to the FE.
  • KDA_Defacto - --3vat sink uses QUAT for sanitizer. QUAT concentration was 200ppm. --Cooked beef patty 174F. --Critical (Critical)
