Observation: Three plastic spatulas are demaged, scraced, dented and no longer cleanable. All three discarded at time of inspection. All utensils need to be maintained in good condition. Corrected on site COS.

Observation: Sanitizer in work container was teste too low at the time of inspection. Must maintain between 100-200ppm for proper sanitizing. Added chlorine to reach the optimum concertation. Corrected on site COS.

Observation: Metal rack is rusty on the one place where is food stored in dry storage. Manager put a plastic mat to cover it. Food contact surface must be clean to sight and touch.Corrected on site COS.

Observation: A light bulb is not working properly in reach in cooler. Must provide adequate light intensity in the area where is food stored inside reach-in refrigerators for easily cleaning and maintaining.

Observation: Floor tiles in following areas have become water damaged and must be replaced: under prep sink in kitchen, under dish machine, and under 3 hole sink.

Observation: Cove base was missing along the wall behind the Groen Steamer. Also cove base in poor condition in same areas that have water damaged floor tiles. Must replace cove base.

Observation: Sanitizer was tested too low at time of inspection. Added chlorine and reached minimum concentration.

Right faucet on 3 hole sink has a leak. Must repair or replace to prevent buildup of bacteria and contamination.
- Drying mops
1) Womens employee RR does not have a covered trash can. Must have a covered trash can in womens RR for adequate disposal of sanitary napkins. 2) Observed mop head stored in mop sink. When not in immediate use must allow mop head to air dry to prevent buildup of bacteria and contamination.

Walk in cooler vent cover has dust buildup. Must keep clean to prevent buildup of bacteria and contamination.

Sanitizer was tested too strong (>50-100ppm) at time of inspection. Chlorine sanitizer must be maintained between 50-100ppm for adequate sanitation. Corrected at time of inspection
