Observation:This is actually for 6-201.11. Walls behind deli and meat dept. sinks are peeling, no longer smooth/nonporous/easily cleanable. Observed peeling paint in meat dept. ceiling. Observed broken floor tiles under deli broaster.

Observation:Observed several foods in meat freezers and display cases, dried fruit in produce, deli dips and baked goods that do not have ingredients and allergens on labels

Observation:Observed coats and purses stored in various locations - there are no lockers or coat hooks

Observation:Observed wood countertop at produce is used as cutting board and is heavily scored, no longer smooth/nonporous/easily cleanable
