Observation: Several cracked plastic container above three sink in the kitchen. Discarded at the time of inspection. All utensils need to be maintained in good condition. Corrected on site.

Observation: 1) The pastry case does not have the large sticker posted on the case stating the ingredients in the pastries/donuts. Must obtain ingredient labels for allergens and post on the pastry case. 2) Ice has no store number and label in ice- cooler. Must have store number or label.
- When to wash
Observation: Employee returned to work after restroom and put the gloves on. Employee stated the washed hands n restroom. Ensure to wash the hands before putting gloves when handling food. Dirty hands may contaminate the gloves if not washed. Corrected on site.
- Storage or display of food in contact with water or ice
Observation: 1) Ice buildup in chest freezer with ice cream in the store. Must defrost to prevent source of contamination. Must keep freezer free of ice. 2) Excessive build up in walk-in freezer and ice on the food. Must keep freezer free of ice, call for service.

Observation: Most recent inception report was not posted in visible level to consumers. Must post the most recent inspection report in area easily readable by the public. Corrected on site.
- Handwashing cleanser, availability
Observation: Paper towel dispenser was empty in the kitchen. Must provide disposable towels or heated-air hand drying device to every handwashing sink. Replaced and corrected on site.
- Food storage containers identified with common name of food
Observation: All seasoning shakers ( paper, salt etc.), and squeeze bottles need to be labeled with common name of the food in the kitchen. Label or use marker. This is repeat violation.

Observation: The working container with hot-dogs an container with Parmesan cheese have no date marked in walk-in cooler. Must date mark all TCS (time/temp control for safety) food with date opened/prepared, or discard ready to eat, prepared on-site or opened commercial container held at 41°F for 7 days. Removed and corrected on site.
- Equipment location, installation, repair, and adjustment
Observation: Condenser discharge line in walk-in freezer is dripping on food boxes. Discard food and make necessary repairs. Call for service.

Observation: The following non-food contact surfaces have food particles or debris needs more frequent cleaning: 1) both reach-in coolers has food residue, 2) microwave in the kitchen and 3) walk- in freezer. Clean more frequently to avoid build -up.

Observation: The walk in cooler had plastic from drinks on the floor and a large amount of onion peels. The food manager swept the floor in the walk in cooler during this inspection.CORRECTED ON SITE.

Observation: The following had numerous types of food items on the floor of the walk in cooler/freezer. ALL of the food was placed at least 6" above the floor during this inspection. CORRECTED ON SITE.

Observation: 1)A bucket of sanitizer was not labeled. ALL chemicals taken from original containers shall be identified with the common name of the chemical.2)A bucket of sanitizer was stored on the floor. ALL sanitizer shall be stored off the floor. BOTH WERE CORRECTED ON SITE.

Observation: One employee observed wearing gloves touching her clothing and not washing hands and changing gloves. ALL employees were educated and verbalized understanding. CORRECTED ON SITE.
