    Observation: Not using test strips to check chlorine sanitize solution in spray bottle. Sanitizer solution was too strong over 400ppm. Made fresh one checked with strips to reach 50-100ppm and corrected on site.

    Observation: Walk-in freezer outside has been broken since summer. All food kept in High school and transported daily to this school as needed it.
  • Foods are cooled using appropriate methods
    Observation: Cooked beans (temped 145*F) stored in deep pan on the table to be cooled and stored in cooler. Manager corrected by placing bean in three shallow pans and will make sure that proper method of cooling will be followed.

    Observation: Opened canned food stored in the original can container such as berry jelly (other souces such as tomato sauc). Galvanized metal may not be used to store any acidic foo d residue in galvanized can.Must store in anither plastic container label with date marking.Remove or discard it.
  • Food temperature measuring devices are calibrated in accordance with the manufacturers specifications as necessary to ensure their accuracy
    Observation: Thermometer in two door reach in cooler show 57*F. Food temperature measuring devices must be calibrated or adjusted in accordance with manufacturer's specifications as necessary to ensure their accuracy. Staff using plastic thermometer that shows 41* all the time. Adjust or calibrate thermometer.
  • Safe, unadulterated, honestly presented
    Observation: Dented cans stored on dry goods shelf. Must discard any can that is dented on top, bottom, or side seals. Also any can that cannot sit level on shelf without wobbling. Corrected on site and removed.
  • Outer openings are protected
    Observation: Exit delivery door continues to have light showing and doesn't shut off automatically. All outer openings must be protected from the entry of insects, rodents and pests. Contact with pest control company for proper treatment or fix it do door.
  • PHF/TCS foods maintained at 41°F or below, except during preparation, cooling, cooling, or when time is used as a public health control
    Observation: Food in cold hold in two doors reach in cooler has temperature at 45*F,47*Find 56*F using three thermometers at three different places. Log sheet said 33*F at 7:00AM. Potentially hazardous food (PHF) Time/Temperature control for safety food such as mayo, sour cream removed as well can of blue berry jelly. All other food was removed to another cooler that must be maintained at 41*F degree or bellow. Manager called the maintenance office today to check the temperature in cooler and send us invoice. Must not use for cold hold until is fixed after holiday.

    Observation: The PSI for the warewashing dish machine exceeds the range per the dish machine PSI control.The flow pressure of the fresh hot water SANITIZING rinse in a WAREWASHING machine, as measured in the water line immediately downstream or upstream from the fresh hot water SANITIZING rinse control value, shall be within the range specified on the machine manufacturer’s data plate and may not be less than 35 kilopascals (5 pounds per square inch)REPAIR AS NEEDED.

    Observation: Two spray bottles of bleach water are not labeled. All working containers of chemicals taken from original packaging SHALL ALWAYS BE IDENTIFIED WITH THE COMMON NAME. CORRECTED ON SITE.
  • Outer openings are protected
    Observation: The exit delivery door in the dry storage area CONTINUES to not close all the way with the current door closure. SHALL REPLACE OR REPAIR DOOR CLOSURE. ALL OUTER DOORS SHALL BE SELF CLOSING AND TIGHT FITTING TO PREVENT ENTRANCE OF PEST AND RODENTS. THIS IS A REPEAT VIOLATION.
  • Food storage - preventing contamination from the premises
    Observation: A box of canned peaches stored on the floor in the dry storage area. ALL food items shall ALWAYS BE STORED AT LEAST 6" OFF THE FLOOR TO PREVENT CONTAMINATION. CORRECTED ON SITE.

    Observation: The upper portion of the microwave has food build up. Shall be cleaned as often as necessary to keep clean. CORRECTED ON SITE.
  • Sanitizers
    Observation:The spray bottle of chlorine sanitizer was OVER 200 PPM. THE COLOR OF THE TEST STRIPS WERE BLACK. This bottle of chlorine sanitizer was discarded and NEW chlorine sanitizer was prepared and is 50-100 ppm. CORRECTED ON SITE.
  • PHF/TCS foods maintained at 41°F or below, except during preparation, cooling, cooling, or when time is used as a public health control
    Observation: The two door upright cooler has a temperature of 49 degrees USING THREE DIFFERENT THERMOMETERS. The food establishment uses a food log sheet and at APPROXIMATELY 07:00am was at 34 degrees per log sheet. At this time within 4 hours all PHF food was moved to another cooler which is 41 degrees or lower. SHALL HAVE THIS UNIT BE 41 DEGREES OR LOWER PRIOR TO USE. Star refrigeration on site to check this cooler. RECEIVED LETTER OF CORRECTION AT THIS TIME AND THE FOOD MANAGER WAS INFORMED BY STAR REFRIGERATION TO LEAVE THE COOLER OFF FOR TODAY TO MELT THE ICE BUILD UP. INSPECTOR SPOKE WITH THE SERVICE PERSON FROM STAR REFRIGERATION AND STATED DUST BUILD UP AND THE EVAPORATOR WAS FROZEN.
  • Drying mops
    Observation: Two mops were observed in mop water. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, EQUIPMENT, or supplies. Janitor was educated and verbalized understanding. CORRECTED ON SITE.
  • Posting of a valid license
    Observation: The most current license HAS expired. The file has the NEW license. The manager made a copy post this until the license with the gold seal and green trim is mailed to the school from Linn County Public Health. CORRECTED ON SITE. The expired license was posted but NOT visible to the public. Manager plans on posting in a more conspicuous place for consumers to view.
  • Handwashing signage
    Observation: The handwashing sink in the kitchen does not have a sign posted when food employees shall wash their hands. Manager has two and posted during this inspection. CORRECTED ON SITE.
  • Posting inspection reports
    Observation: The most current inspection is posted but not visible for the public. The manager plans on placing in a conspicuous place for consumers to view.

    Observation: The exit door in the dry storage area has been repaired for any light shining through. The door at this time will not shut all the way with the current door closure. Repair or replace the door closure as needed. Exit doors shall be self closing and tight fitting to prevent entrance for rodents or insects.(During this inspection very windy day re-check in spring or when not windy and repair if needed.)

    1) Observed a sponge in dish room. Sponges may not be used in contact with cleaned and sanitized or in-use food-contact surfaces. Discarded. Corrected at time of inspection. 2) Observed several damaged utensils. All utensils must be maintained in good condition to prevent contamination buildup or bacteria. Replace.

    Dish soap items were stored above single service items in larger dry storage by laundry room. All chemical items must be stored separate or below food single service items.
  • Handwashing signage
    Hand washing sign not posted in men's restroom. Every hand sink to be used by food employees must have sign posted reminding employees to wash hands.

    1) Observed a sponge in dish room. Sponges may not be used in contact with cleaned and sanitized or in-use food-contact surfaces. Discarded.     Light bulbs were not shielded in the small dry storage which has milk cooler in it. Must be shielded or replace to shatter-resistant in areas where there is exposed food.

    Exterior door in larger dry storage by laundry room has light showing around outer edges. Must replace weatherstripping or door sweep to prevent entry of pests.

    Overall have great date marking. One student has special request for milk. Must date mark milk bottle if not used within 24hr after opened. Have 7 days from date mark to use or discard. Do not use the sell date on milk bottle as expiration date. Corrected at time of inspection

    Has toweling over clean trays, must provide a nonabsorbent material for this purpose, such as food grade bag to separate toweling from food contact surfaces.
  • Food storage - preventing contamination from the premises
    1) Has case of potatoes on floor in kitchen. store at least 6 inches off floor. 2) Has many food boxes stored on floor in walk in freezer (under the racks) and on shallow wooden board in dry storage area. Need more shelving units to be able to store all foods at least 6 inches off floor to aid cleaning.

    1) Has dish soap stored on shelving with food and food service supplies, must move to area below and not directly adjacent to foods and food service items. 2) Has blue tub for wiping cloths, must label contents to help identify contents. Both corrected on site.

    Door sweep is worn in center and light showing through at doorway to exterior walk in freezer, replace/repair door sweep to prevent pests from entering.

    License is not posted in public view, suggest move inspection to area more accessible to be seen by public, as well, ie milk cooler wall.
