    Observation: Box with bread sticks stored on the floor in walk-in cooler. All food must be kept at least 6" off the floor. Correct on site.COS

    Observation: There is a whole on the wall next to the customer's restroom. Physical facility must be in good repair to prevent entry of pest. Cover or repair. --Observation: A missing one ceiling tile in employee's restroom. Manager put the tile. Corrected on site. COS.

    Observation: License in not visible to the consumer and posted on the board next to entrance to the kitchen.The current license is posted on a board but not visible for consumers to view. Move the license in a conspicuous place for consumers to view.Corrected on site. COS.
  • Food is properly labeled
    Observation: Donuts/pastry cookies have no labels information for allergy information for sale at store display. Food must be labeled with common name of the food and major food allergens.Corrected on site. COS.

    Observation: Missing light bulb in reach-in pizza cooler. Must provide adequate light intensity in the area where is food stored inside reach-in refrigerators for easily cleaning and maintaining.

    Observation: : Ice buildup in chest freezer with ice cream.. Must defrost to prevent source of contamination. Must keep freezer free of ice.

    Observation: Overall date marking is good but one container with gravy not date marked after opening in reach-in cooler. Potentially hazardous food held in food establishment for more than 24 hrs. must be clearly marked with date made, consume by, or sell by, and be held at a temperature of 41*F or less, for no more than 7 days from day marked.Corrected on site. COS.
  • Plumbing system maintained in good repair
    Observation: The following plumbing fixtures are not in good working order and must be repaired: 1) the sink faucet on three compartments sink is leaking, 2) hand sink pluming is leaking in the kitchen. All equipment must be in good working order. Repair.
  • Food storage containers identified with common name of food
    Observation: Overall labeling is good but one container with gravy sauce has no label or date. All food items taken from original packages shall be identified with the common name of the food.Corrected on site. COS.

    Observation: Both RS/FE licenses are posted but NOT in a conspicuous place. SHALL post both licenses in a conspicuous place for consumers to view.
  • Physical facilities maintained in good repair
    Observation: The employee restroom door does not close properly. The owner plans on replacing this door within 30 to 60 days. The restroom door shall be self closing and tight fitting. Send in letter of correction after the door is installed by July 8, 2014.
  • Storage or display of food in contact with water or ice
    Observation: One box of pizza crust on the floor in the walk in freezer. SHALL ALWAYS keep food at least 6" off the floor. CORRECTED ON SITE.
  • Miscellaneous sources of contamination
    Observation: Some open boxes of various food items in the walk in freezer were not closed to prevent contamination. After opening food items SHALL ALWAYS keep covered to prevent contamination. CORRECTED ON SITE. --Observation: The reach in ice cream cooler has build up of ice and needs to be defrosted. Owner plans on defrosting when more of the ice cream is sold to keep from having too much ice cream in the walk in freezer.
  • Light bulbs, protective shielding
    Observation: The walk in cooler at the entrance has a bulb not working and the glass cover is cracked. This cooler has sufficient light with the other 4 light bulbs attached to the ceiling of the cooler and are all shielded. The owner stated he may just remove this light due to the cooler has sufficient lighting without it. SHALL either REPLACE/REMOVE THE CRACKED GLASS COVERING.
  • Sanitizers
    Observation: Spray bottle of Quat sanitizer was in excess of over 200 ppm. SHALL maintain Quat sanitizer at 200 ppm at all times per manufacturer label. Suggested to owner to make sanitizer in the 3 tank sink and then obtain sanitizer for spray bottles. CORRECTED ON SITE.
  • Storage of clean equipment and utensils
    Observation: One box of disposable cups were stored on the floor. SHALL ALWAYS keep single service items at least 6" off the floor to prevent contamination. CORRECTED ON SITE.
  • Designated areas for employees

  • Food temperature measuring devices are provided and readily accessible
    Observation: The reach in pizza preparation cooler does not have a temperature measuring device. ALL cooling units shall be provided with thermometers. CORRECTED ON SITE.
  • Posting inspection reports
    Observation: The last inspection was posted but not at eye level and difficult for consumers to view. SHALL POST this inspection in a conspicuous place for consumers to view.
  • Equipment location, installation, repair, and adjustment
    Observation: The upright cooler with various food items on the bottom shelf has standing water.(condensate) The cooler has a sign posted to remind staff to wipe down the cooler every day. Owner needs to call a certified refrigeration service to verify the water source, due to the cooler has standing water. Owner also stated due to age of this cooler this cooler might be replaced in the near future.
  • Food storage containers identified with common name of food
    Observation: Several plastic containers of frosting for the donuts are not labeled. When removing food items from original packaging shall identify the common name of the food item. CORRECTED ON SITE.
