- The counters in the drive through area and the front are damaged/missing laminate.
- The chlorine sanitizer bucket in the front was low ~10ppm. It was remade. Also the sanitizer in the 3-compartment sink was made with hot water. Please go over how to properly make sanitizer with employees.
- The Freeze machine is leaking syrup on the counter. The ice machine drip tray has black buildup. The cabinet under the soda fountain has debris.
- The floor under/around the fryer has buildup on it.
- The wall by the mop sink is damaged and missing a coving tile. There is a ceiling tiles missing over a duct.

- The cabinets under the soda fountain in the dining room appear to have had some water damage. One doesn't seem to even close properly, it hangs astray.
- Under the soda fountain in the dining room there is an accumulation of debris and buildup.
- There are coving tiles missing by the mop sink. Also there are tiles missing by the freeze machine.
- There is a light shield with the corner missing by the assembly line. Also many of the ceiling tiles are stained making them appear dirty.

- The cabinets under the soda fountain in the dining room appear to have had some water damage. One is held shut by tape and the other when you open it falls lower.
- Under the soda fountain in the dining room there is an accumulation of debris and buildup.
- The hand sink by the 3-compartment sink leaks.
- There is tiles missing by the freeze machine.
- There is a light shield missing in the back and another that has a corner missing. I pointed this out to the manager.

- Found food build up on shelves, hot holding wells, cold holding wells. Clean and sanitize these areas.
- Found build-up on handles of cabinets, coolers, and freezers. Clean and sanitize them.
- Found a KAY QUAT bottle with bleach in it. Per manager, this facility does not use spray bottle for sanitizing. He disposed of it.
