- Observed cooked and cooled ribs, turkey, mashed potatoes in the walk in cooler that were only labeled with the date. Label with both the date and time of onset of cool down. The times were recorded on cool down charts.
- Observed the dish washing employee not washing their hands between handling clean and dirty dishes, re-contaminating clean dishes. Employee was retrained immediately by manager and inspector on when, why, and how to properly wash their hands.
- The cap was missing off of the back flow prevention devise on the back of the mop sink faucet in the mop room connected to the kitchen. Repair it.
- Observed fruit fly killer stored with cleaning chemicals in cabinet under the hand sink in the main dining room. These items must be stored separately. Corrected--Manager moved the pesticide to a different storage closet with no chemicals.

- Observed employee handle dirty dishes to put into the dishmachine, then put away clean dishes without washing hands in between. Also employee wiped hands on rag that was on the 3-compartment when her hands were wet. Also she touched face and wiped her nose with her sweatshirt between handling clean dishes. There was no hand washing of any kind. This is a training issue. You need to conduct a training on hygiene and proper hand washing. Once you have done the training please email me with the details of the training and how many employees attended. I need this by 12-15-14.
- Observed 2 rubber spatulas that were frayed on the edges. Suggested trimming areas with damaged pieces. Also there were a few that were stained.
- Observed duct tape being used to seal the bottom of the clear ice bucket.
- The hand sink in the dining room had a sanitizer bucket in it. The bucket was empty. The bucket was removed. Discussed not keeping anything in or around the hand sink.
- Observed carpet rug being used on kitchen floor in front of back door.
- Observed small area of coving missing on the middle island counter in the kitchen under the fire extinguisher.
