- Observed soda lines sitting on the floor in the area across from the walk in freezer. Tie soda lines at least 6 inches off the floor to protect from contamination. Corrected.
- The manager was not wearing an effective hair restraint. All employees working with or around food must wear a hat, hairnet, or other hair covering. Corrected.
- Replace damaged, black caulk along the 3-compartment sink/wall juncture and the mop sink/wall juncture.
- The fan vent cover in the walk in cooler, walk in cooler door seal, and the sides and in between the cabinets in the service line area had a build up of debris. Clean and sanitize these areas.
- Repair or replace the damaged piece of base cove on the South wall under the storage shelf.
- The floors under, around, and behind equipment and near the back door had a build up of debris. Clean the floors. Corrected.
- The walls throughout the facility (kitchen, behind water heater, prep area) had a build up of debris. Clean the walls.

- Observed employee wash hands and then turn of the faucet with her bare hands. I retrained the employee immediately on proper handwashing procedures and she demonstrated proper handwashing techniques.
- Observed tear in lower cabinet where single service items are stored. Crumbling brick wall is behind tear. Repair or replace. Observed rusted metal rack near back door of facility where chips are stored. Observed the coating on metal rack above 3 compartment sink that is peeling. Observed metal rack across from walk in cooler that is rusting. Reseal or replace these items.
- Observed debris accumulation on the following items/surfaces: outer surface of walk in cooler and walk in freezer, Cabinet beneath soda machine in dining room, Soda machine in dining room, Cabinets beneath microwaves, Inside the backup prep cooler, Metal racks that hang (across from walk in cooler), and the Metal chip rack by back door. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.

- Found build-up on handles of microwaves on the prep line. Also there is black buildup behind the 3-compartment sink in the grout.
- The floor tiles by the back door are cracked. Per manager the floor tiles are no longer available and therefore can't be replaced. Discussed using grout or similar method to ensure the cracks can't collect water and debris.
