- Observed container of pepperoni labeled 12/9 in reach in cooler. Per manager, she probably, mistakenly put the wrong lid with the label on it on the container. Manager voluntarily discarded pepperoni. Ensure all food labels are verified to be correct and are used by or discarded 7 days after the first day of prep.
- Observed soda lines near floors in the kitchen. Manager secured soda lines so they were no longer on floor. Observed several boxes of iced coffee, creamer, and other drinks stored on the floor in walk in cooler. Also observed 3 frozen pizzas on floor in walk in freezer. Ensure all food and drinks are stored 6 inches off floor.
- Observed employee foods being stored in walk in freezer next to pizza. Manager moved employee food to lowest shelf away from facility's food. Ensure there is clear, physical separation from restaurant food and employee food.
- Observed broken handle on right side of 2-door freezer. Repair handle. Also observed rusting metal shelves for alcohol storage. Repaint or replace metal shelves. Can consider Rustoleum spray can which prevents metal pieces from shedding off surfaces. Corner prep sink faucet is slightly loose and sink is pulling away from wall; reseal sink to wall.

- Floors, especially in corners, under shelving units in kitchen and display area, as well as walk-in cooling units throughout the facility have build up of debris and dried up liquid spills. Observed dead insects on floors in multiple locations, especially in dry storage areas. Clean floors.
- Observed kitchen to be dirty in the 3 compartment sink area of the kitchen. Clean.

- Observed hand sink on the front line coming apart from wall. Repair and caulk along the top edge. Laminate coating in disrepair under soda machine's backside counter. Repair. Rusty metal racks in back. Gasket to the first walk-in cooler broken. Repair.
- Observed food debris build-up in the utensil drawer. Wash, rinse and sanitize.
- Observed handles soiled to the bottom door of the oven/warmer. Handles soiled to the 2-door reach in freezer. Gaskets soiled to the 2-door reach in freezer. Several equipment in the back has food and dust build-up. Wash, rinse and sanitize all these items.
- Observed single serve pizza boxes box stored on floor. Two boxes of J Cups stored on the floor in the back room. Store boxes six inches off the floor.
- Observed dust build-up on vents above prep sink. Clean.
- 6 TCC 3-4 (a) Facility unable to locate TCHD 2015 permit. Facility has 10 days to locate and post to the public. A re-inspection will occur after that date.

- Wood becoming exposed behind hot holding unit. Duck tape on counter by cheese dispenser.
- All continuous use food contact surfaces have to be washed, rinsed, and sanitized. These surfaces include but are not limited to the soda machine nozzles and inside the cheese dispenser. The cheese dispenser had dried cheese inside the machine.
- All non-food contact surfaces have to be washed, rinsed, and sanitized. These surfaces include but are not limited to under the doughnut case, cabinets seals, cabinet shelves, cabinet/cooler/freezer racks, under the lid holder by the soda machine, and the hot chocolate dispenser grate. The areas listed had a lot of build up on them. The lid holder by the soda machine had a brown liquid looking stain under it. The lids did not appear to be touching the liquid.
- Peeling paint on walls in the kitchen and soda dispenser area.
- Ceiling vents throughout kitchen have dust build up. Build up on ceiling tile frames throughout kitchen. Fuzzy build up on ceiling in cooler and freezer.
