- Observed an open can of Redbull on dishroom counter near onion slicer. Open containers of onions were nearby. Ensure employee drinks are stored separately from food and food contact items. Also ensure employee drinks have a lid and straw to prevent contamination of hands then other surfaces. Discussed this with manager during inspection. Drink was removed.
- Observed cigarette ash tray with several cigarettes in them in garage/dry storage room. Per manager, employees who smoke open the garage and smoke in that room. Discussed with manager, smoking is not permitted inside restaurant facilities and employees must be a minimum of 15 feet away from doors, windows, or ventilation intakes. Manager discarded cigarette ash tray.
- Observed broken gaskets on Pepsi cooler in cookline. Repair or replace broken gaskets. Also observed unsealed wood in garage/dry storage room. Seal wood, with either paint or other sealant.
- Observed moist wiping cloths sitting on prep top cooler. Ensure wiping cloths used for sanitizing surfaces are stored in sanitizing solution in between use to prevent bacterial growth.
- Observed food buildup on inside, front surface, and handle bars of cookline microwaves. Routinely clean these cooking surfaces.
- Employee restroom door is not self closing. Install a self closing latch.
- Handsoap was empty in employee's restroom. Replenish soap.
- Observed grease and dust buildup on vents above fryers. Per manager, they are schedule to be deep cleaned very soon by a professional crew.
- Observed missing light shield on lamp above fryers. Install missing light shield.
- Chlorine sanitizer in spray bottle wash greater than 200 ppm. Test strip was almost bleached out. Employee emptied bottle and refilled it with appropriate chlorine concentration. New concentration read 50 ppm. Ensure chlorine concentration remains around 100 ppm for sanitizing spray.

- Observed two gallon jugs of milk without date label. Per the code milk should not be dispensed for single use.
- Observed a container of soup in the walk in cooler with out a label. The container was thrown away upon inspection.
- Observed rust in the walk in cooler. Seals pointed out to the manager throughout the facility had black debris build up. The chest freezer against the garage has a broken seal.
- Observed 3 compartment sink in the kitchen leaking. Repair.
- Observed the floors throughout facility especially in the dish wash room around and around equipment near the fryer and by coving.
- The juncture between the 3 compartment sink and the wall in the kitchen has black debris build up. Around the dish machine has black debris build up. Wash, rinse, and sanitize these areas.
- Observed extraneous items being stored on the shelving in the back by the ice machine.

- Observed an open employee cup of ice water on the shelf with single-service items. Corrected-- Drink was disposed of by manager and staff was retrained.
- Observed unfinished particle board shelves in the dry storage area and inside wood cabinets in the waitstaff area. The door to the walk-in cooler and tops of the white chest freezers are chipped and rusty. Paint these areas so they area non-absorbent and easily cleanable.
- The outside of the pizza oven had a build up of debris. Clean and sanitize it.
- The staff restroom does not have a self closer.
- The walls and ceilings in the beer walk in cooler were rusty. The ceiling in the dry storage room was open with exposed rafters and insulation. Repair or cover/paint these areas.
- Found chlorine spray bottle at >200 ppm. Maintain at 100-200 ppm. Corrected to 100 ppm.

- Found unlabeled containers of breading and flour.
- Observed sugar scoop (dish washing room) and ice scoop (server station) stored in food product.
- Observed rust on lid of white deep freezer in back storage room with the garage door. Repair.
- Dish machine thermometer not properly working.
- Wash, rinse and sanitize non-food contact surfaces regularly throughout the day. Observed several surfaces in the cooks line soiled. Also, observed handles to kitchen equipment and cooler handlers with food debris build-up.
- Rust is starting to form on walls inside keg cooler and metal shelving. Also, observed wall paneling sealer melted from existing oven in the cooks line. Replace sealing paneling.
- Missing light shields to the lights in the back storage room with the garage door. Provide light shields.
- Observed cleaning equipment not properly stored in cleaning/mop storage room.
