- The following items/areas were observed to be broken: the True refrigerator door seal across from the grill, the Cool Beverages cooler door seal, and handles of the Superior refrigerator. Replace. Also, the white shelving unit in the back dry storage room was observed to be chipping off. Reseal. The beer cooler behind the bar has water accumulation at the bottom that is causing the unit to be rusty.
- There is buildup on the knife storage, can opener, the soda gun nozzle holder (in the bar area), and the black cart with spatulas on the side.
- Buildup on the following: storage shelf under grill, storage shelves of prep tables, door and seal of True Refrigerator, fan by hand washing sink, seal and door of Superior Refrigerator, the track of the doors of the beverage cooler with clear doors, top of ice machine drip tray and door, black cart by ice machine, walk in cooler and beer cooler door seal, and both coolers in the bar area.
- The glasses were stacked together wet in wait staff area.
- The facility reuses single service containers. These are not meant to be washed, rinsed and sanitized.
- There is buildup on floors in corners and around equipment throughout kitchen. There is so much in storage it hard to clean under shelves and around equipment.
- Food damaged/missing floor tiles behind shelf that has buckets on it, in the beer walk in cooler, and under the beverage cooler with the clear doors.
- There is a light in the kitchen that is missing an end cap. There is also tape on some of the light shields that is coming off.
- There are many extraneous items in the kitchen such as old cash registers. There are signs on the registers used for parts. But there are multiple old registers. There is also areas where there is stuff from the floor to the ceiling. Also there is stuff on top of the walk in cooler/beer cooler.
- Employees still haven't gotten their food handler's licenses.

- The facility's metal stemmed thermometer had broken so they were using the large meat thermometers.
- The reach in cooler on the prep line has a broken handle.
- The hood and equipment area had an accumulation of grease. Also the bottom of coolers and freezers had food buildup.

- The employees haven't gotten their food handler's licenses yet. Explained what it is to the employee in the kitchen and how to get it.
- The facility had 6 containers of cooked and cooled down pizza meat. The cook said that pizza meat is a mixture of sausage and hamburger. I explained that as a category 2 you are unable to cool anything down. You will either have to cook the meat each day or buy precooked meat for pizzas. The 6 containers were voluntarily disposed of.

- Found chipped white shelves in back pantry. Non-food contact surfaces must be smooth, durable, and non absorbent. Reseal or replace.
- Found the concentration of sanitizer in 2 sanitizer buckets to be between 1-10 ppm. Maintain bleach sanitizer in bucket at 100-200 ppm. Sanitizer concentration was corrected on site at time of inspection.
- Found the following items and surfaces to have an accumulation of debris: Cooler with salad items, white freezers, fryer filter machine, hand sink in bar, all carts in kitchen, and equipment; grill, stove, fryer. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
- Found the floors throughout facility missing tiles. Repair or replace.
- Found floors throughout kitchen with an accumulation of debris buildup. Clean floors and clean walls throughout kitchen especially by 3 compartment sink, grill and fryer area, and back cook line. Ensure routine cleaning.
- Found ethanol cans stored above food. Chemicals shall be stored separately and below food and food items. Ethanol cans were moved at time of inspection.
- Found unlabeled spray bottle. Ensure all chemicals and bottles are properly labeled. Bottled was labeled by employee at time of inspection.
