- Observed employees not wearing effective hair restraints. Anyone in the kitchen and doing food prep needs a hat.
- There is a drip on the faucet at the hand sink by the office. There was also a drip under the 3-compartment which the manager fixed.
- The hand sink by the office was blocked by a broom and dust pan. I had to move both in order to wash my hands.

- Observed rusty fryer baskets. Ensure all food contact equiptment is in good repair.
- Observed broken seal in cooler under grill presses. Repair.
- Observed debris accumulation on table top near oven. Ensure all surfaces are clean and sanitized.
- Observed debris accumulation on ovens. Clean and sanitize these areas on a regular basis.
- Observed containers stacked wet on drying racks near dish washing station. Ensure all items are completely air dried before stacking.
- Observed paper towels missing from hand washing station near three compartment sink. Paper towels were provided at time of inspection.
- Observed juncture between wall and hand sinks missing caulking. The juncture should be sealed and easily cleanable.
- Observed missing ceiling tile in the mop room. Ensure all ceiling tiles are inn place and easily cleanable. Also, observed a hole in the wall on the North wall in the utility room. Repair the wall.
- There has been a change in management recently. Currently, only 2 Certified food manager (Fssmc) certificates were available on site. At the time of the inspection, no FSSMC certified managers were at the facility. As a Category 1 facility, a FSSMC certified food manager must be at the facility during all times of food preparation and service. Get more people certified. - Observed employee wash her hands and then turn off the faucet with her hands, recontaminating her hands. Turn off the faucet with your paper towel after drying your hands to prevent recontaminating your hands. Conduct a training with staff and submit documentation to TCHD, Attn:Kim by 4/2/15.
- The grills and other cooking equipment had a build up of debris. Clean and sanitize them.
- The insides of the coolers had a build up of debris and the Groen oven was dripping. Clean and sanitize these items.
- The outside of the under counter cooler that is not working/used for storage and the hot holding cabinet had a build up of debris. Clean and sanitize these areas.
- The lids to the cardboard and grease dumpsters were open.
- Found build-up of debris on floors. Clean the floors under and around equipment and in the walk in cooler.
- Found food build-up on walls. Clean the walls.
- Wet mops were stored in the mop sink. Hang them up to dry.
- No certified food manager was present at the time of the inspection. As a category 1 facility, a certified manager must be at the facility during all hours of operation.
