- Found hot dogs, polish sausage, cheeseburger bites on rostissere cooker at 108F. Maintain hot foods at 135F or higher. Per manager, these foods went on the cooker at 7 a.m. and were still cooking. A still cooking sign was put up.
- Both the large walk in freezer and the small walk in freezer had a large build up of ice. Remove the ice build up from the freezers.
- Kitchen employees were not wearing hats or hairnet. Provide and utilize. proper hair restraints.
- Repair or replace the cracked door seal to the large walk in cooler.
- Found wiping cloths stored on the counter next to the hot holding unit and by the beck of the Big Freeze machine. Store them in sanitizer between uses.
- The inside of the pizza prep cooler, both soda machine ice dispensers and nozzles, inside of the True refrigerator had a build up of debris. Clean and sanitize them.
- The pizza oven, lobby microwave, toaster oven in the kitchen had a build up of debris. Clean and sanitize them.
- The can opener blade and insides of the cabinets in the lobby had a build up of debris (especially under the dump sink). Clean and sanitize them.
- The 3-compartment sink, outside of the True refrigerator, door seals to the walk in coolers and freezers had a build up of debris. Clean and sanitize these areas.
- There were 2 large boxes of cups stored on the floor in the storage area in the kitchen. Store them at least 6 inches of the floor.
- The three compartment sink faucet was leaking from the base. Repair it.
- Repair or replace damaged, cracked, and missing flooring under the prep sink by the 3-compartment sink and in front of the walk in cooler and large walk in freezer.
- Found build-up of debris on floors. Clean the floors, especially under and around equipment. Clean up spill under the milk shelf in the drink cooler and walk in freezer floor.
- Found food build-up on walls behind the 3-compartment sink. Clean the walls.
- Replace the burnt out light bulb in the drink walk in cooler.

- Found 2 boxes of food stored on the floor in the walk in freezer and a couple of soda lines laying on the floor under the boxed soda. Food was immediately put on shelves in freezer.
- There was larges ice build up on some boxes of food in the walk in freezer.
- Found wiping clothes stored on counters. Store in sanitizer pail between uses.
- The pizza oven and grill had a build up of debris. Clean and sanitize them.
- The soda machines had a build up of debris on them. Clean and sanitize them.
- The outsides of food containers in the pizza fridge, walk in cooler door seals, cooler handles, and cabinet under the sink in the lobby had debris build up on them. Clean and sanitize them.
- Backflow devise at the mop sink is leaking. However, there was an air gap between the hose and the top of the mop sink. Repair or replace the backflow devise.
- Found damaged floor tiles outside of the walk in freezer. Repair or replace them..
- Found build-up of debris on floors. Clean the floors.

- Prepackaged brownies and PB&J sandwiches did not bear a label with specified information. Please make sure that prepackaged food set out for customers has a label that includes the following description listed above.
- Some of the soda lines on the bottom shelving were less than 6 inches off the floor. Suggest rewiring the lines to avoid them from touching the floor. Also observed some cardboard boxes of cups being stored on the floor
- There was food debris build-up on the lids, and around the containers that were used for cold-holding pizza toppings. Suggest washing, rinsing, and sanitizing on a regular basis to avoid excessive build-up. Please clean inside toaster oven and tongs. Fix clogged drain for soda machine near drive-in window and wash, rinse, sanitize dispenser nozzles.
- Please wash, rinse, and sanitize the following: Metal table across from ice machine where black liquid had settled, around water filter system, door handle for walk-in cooler containing pizza trays/ door handle for cold holding pizza toppings cooler, hand-sink, interior of 3-compartment sink, cabinet below customer hand-sink (next to coffee machine).
- Clean baby changing station in women's bathroom.
- Observed 2+ dead insects behind ice machine. Clean area behind ice machine, suggest utilizing insect traps and monitoring area weekly.
- Damaged floor tiles in front of walk-in coolers next to ice machine.
- Please clean the following: Black build-up on walls/ floor around ice machine, trash and spilled/ dripping liquid below soda lines, spider webs in space between hot holding pastries and coffee station.
- Dirty vents and dust collected on lighting fixtures above pizza cutting station in kitchen.
- Small gas can, shovels and bucket of de-icing salt being stored in kitchen area. Corrected on-site. Suggest storing in equipment closet.
