- There are damaged floor tiles behind the prep counter in front of the dough mixer.
- Found walls in the sink room that appeared to have had water damage. There are also stained ceiling tiles. Some walls have been replaced though.
- There are bags of empty cans almost to the ceiling. You need to recycle them quicker, not wait until they get to the ceiling.

- Found walls throughout the facility that were cracked, peeling, or appeared to have had water damage.
- There is an accumulation of buildup on the walls. On one place in particular people have actually carved words into the buildup. Also behind the 3-compartment sink there is some sort of buildup perhaps dust.

- Found unlabeled items in cooler on make table without labeling. If item is kept for 24+ hrs, it must be labeled and dated.
- FOUND DUST BUILD UP IN THE WALK IN COOLER. CLEAN THE COOLER. Found debris buildup on utensil holder. Clean and ensure routine cleaning.
- Found debris buildup on floors in facility especially around equipment. Clean floors and ensure routine cleaning.
- FOUND WALLS CRACKED AND PEELING IN MULTIPLE SPOTS IN THE FACILITY. WALLS SHOULD BE MAINTAINED IN GOOD REPAIR.
- Found debris buildup on walls and ceilings in facility. Clean and ensure routine cleaning.
