No thermometer was present in front Turbo cooler and smaller black cooler behind front register. Provide a thermometer in both cold holding units. Especially, provide thermometer in small, black cooler to keep track of temperatures. Observed damaged gaskets of 2 door cooler. Ensure gaskets are repaired or replaced. Observed cracked floor tile in small area near back microwave. Ensure all floor tiles are in good repair to be durable and easily cleanable. Observed coffee grounds in lower part of shelving and on floors. Ensure routine cleaning of facility floors. Observed two cans of Raid insecticides being stored near sanitizer under 3 compartment sink. Ensure all sanitizers and stored separately from insecticides. Manager removed insecticides on site. Observed 3-compartment sanitizer to be greater than 200 ppm Chlorine. Instructed manager to empty half the sink and dilute with fresh water. Retested sanitizer and concentration was still above 200 ppm. Instructed manager to empty half of sink again and dilute the water. Final chlorine concentration read 100 ppm. Discussed with manager how the correct concentration for bleach should be 50-100 ppm in 3-compartment sink and 100-200 in sanitizer bucket.	11/16/2015	Regular	86
By the coffee scale there were coffee filters out and the containers labeled frapp and cocoa mix were open. Explained both should be covered and not stored in front of the window. There is a damaged floor tile in front of the coffee scale. Found build-up of debris on floors. Clean the floors. There is a rug in the basement in front of the chest freezer that is badly stained. The employees have not gotten their food handler's licenses. Per employee they are going this month to the Health Dept.	04/01/2015	Regular	95
Found container of ham, turkey, and chicken with expired labels. Potentially hazardous foods must be discarded within 7 days. Food was voluntarily disposed of by manager at time of inspection. Employees were observed without any hair restraints. Ensure hair restraints are used by staff. Found large shelf in kitchen to be rusting and chipping. Reseal or replace. Ensure all surfaces are smooth, durable, easily cleanable, and in good repair. Found the following surfaces and items with an accumulation of debris: Shelves in basement, display fridge, mini fridge behind register, windowsills in kitchen, all cabinets and shelving in kitchen and cashier area, ice machine (inside and outside). Wash, rinse, and sanitize these items/surfaces and ensure routine cleanning. Found dishes stacked wet. Ensure all dishes are completely air dried before stacking. Found floors in basement to have an accumulation of debris underneath racks. Found bathroom floor to have an accumulation of debris. Found floors throughout kitchen to have an accumulation of debris.
