  • Observed evidence of smoking in the basement including cigarette butts and the smoke smell. Owner will tell employees.
  • Observed seals broken in the kitchen cooler under the microwave and the freezer behind the bar holding alcohol and glass cups. Cabinets and shelves in the kitchen were chipping and had exposed wood, re-seal. Door to the men's restroom is damaged. Observed wood under freezer in the dining area.
  • Observed equipment on the grill line with grease build up in the kitchen, especially in between units. The pizza oven behind the bar has food build up on the tray and rack.
  • Observed the faucet for the three compartment sink in the kitchen leaking. There is also a pipe in the basement that is dripping by the floor drain.
  • There is an indentation in the floor of the walk in cooler that has water collected in it. There are also damaged floor tiles throughout the dining room and behind the bar.
  • There is broken glass in the basement outside where the soda lines are.
  • There is some black buildup on the walls in the basement. Also some grease in the kitchen.

  • The cabinets still have exposed wood and laminate wood. Discussed short term fixes versus long term. Owner said if it's not painted it will be replaced, but she needs a couple of months.
  • There's still dripping in the basement. Per owner a plumber will be coming out.
  • The floors are still in rough shape. The owner has put sealant in cracks in the floor. To fix the floor they would have to replace tiles and repair subflooring.

  • Observed eggs being held above cheese, sour cream and other ready to eat foods in the reach in cooler on the left in the basement. The eggs were moved to the bottom shelf upon inspection.
  • Observed a rag and coasters being used under the ice machine. Observed wood under freezer in the dining area.
  • Observed seals broken in the kitchen cooler under the microwave and the freezer behind the bar holding alcohol and glass cups. Cabinets and shelves in the kitchen were chipping and had exposed wood, re-seal. Handle on the oven in the kitchen was missing. The handle on the upright cooler in the basement handle was broken. Wood pallets holding beer in the basement are not sealed. Door to the men's restroom is damaged.
  • Observed equipment on the grill line with grease build up in the kitchen, especially in between units. The pizza oven behind the bar has food build up on the tray and rack.
  • Observed the faucet for the three compartment sink in the kitchen leaking. Drain pipe leaking into bucket in the basement. There is also a pipe in the basement that is leaking by the floor drain.
  • Observed the door to the beer garden floor plate broken. Manager went to buy items to fix the door during inspection.
  • Observed dead crickets in the room with the soda lines in the basement. Observed ants and flies in the kitchen area. Discussed covering food and food contact surfaces from flies in the kitchen. Also utilizing fly strips, but not around food or food contact surfaces. Also discussed proper cleaning to better monitor if there is an ongoing issue.
  • There is an indentation in the floor of the walk in cooler that has water collected in it. There are also damaged floor tiles throughout the dining room and behind the bar.
  • Observed build up on the floor in the following places: behind the wooden storage cabinet in the basement and under equipment in the kitchen. There is also water collecting under soda lines in the basement as well as the Perlick beer cooler behind the bar. This water is not potable so you need to make sure it does not contaminate food or food contact surfaces.
  • Walls in the basement have paint chipping. Bottom of the wall around the office in the basement is black. Possibly from water water damage because it's not sealed. The walk in cooler wall appears to have water damage.
  • There is buildup on walls in the kitchen by equipment.
  • Observed Titebond flooring adhesive behind the bar stored next to cups and above the cooler holding ketchup, mustard, and beer. The adhesive was moved during the inspection.

  • Found foods that had been cooked and cooled down with the date but not time of removal from heat.
  • In the reach in cooler in the kitchen with the clear door there was raw hamburger and raw chicken stored over condiments that had been opened. The meat was moved to the basement.
  • The bartender didn't wash her hands properly. She touched the handles of the faucet when she turned it off. The cook did wash his hands properly. He touched ready to eat foods (lunchmeat) with his bare hands. The facility does not have a bare hand policy. The facility needs to have a glove and handwashing policy personalized to the facility.
  • The handle to the oven is missing. The handle to one of the upright refrigerator units is broken. The seal to the reach in cooler in the kitchen with the clear door is broken. The cabinets in the kitchen used to hold utensils is wood that is dirty and stained. There is a hole in the men's restroom door.
  • There are rags on the prep counter. The cook used one to wipe his hands after washing dishes. Keep rags in sanitizer.
  • The grill and fryer in the kitchen have grease and food buildup. The oven/stove is dirty. The utensil drawers are dirty with food buildup. The ice machine has black on the drip tray. The popcorn machine and pizza oven behind the bar both have food buildup.
  • The 3-compartment sink faucet leaks.
  • There are dead insects throughout the basement. Per cook the facility does have a licensed pest company that treats the facility. Explained that insects come out to die after spraying so they need to clean. The basement needs thoroughly cleaned so you can better monitor the pest issue as it may be a cleaning issue opposed to an ongoing pest issue. I will check back on this when I conduct a reinspection.
  • The floors behind the bar and in the sitting area are in bad shape. The tiles in the kitchen have been replaced though.
  • The floors in the basement by the soda syrup are black.
  • There is unsealed wood walls in the basement. The bottom of the wood is black potentially from water damage or mold. All walls and ceilings need to be easily cleanable.
  • The walls in the basement are black potentially from mold and there is dust accumulation in the walk in cooler.
  • The lights in the kitchen aren't shielded.
  • There is a gas can in the basement. The gas can is stored by an old microwave and hot holding unit. The gas could potentially contaminate the equipment. Someone needs to go through what's in the basement and get rid of anything that's not necessary. The gas can wasn't moved at time of inspection due to having things around it and it was hard to access. When I conduct a reinspection I will verify this was moved.
  • The basement is full of extraneous items. This allows pests a place to hide. Anything that has not been used within the past year needs to be removed.
  • The new owner is in the process of making a new HACCP. I will work with him on making the HACCP personalized to the facility. They also haven't been doing temp charts. They have blank ones to start using. I will check on the HACCP and temp charts when I conduct a reinspection.
