- Found 2 unlabeled bags of cooked corn beef and 1 unlabeled container of chili in cooler. The manager voluntarily threw the food out. All RTE potentially hazardous food should be labeled and used/discarded within 7 days.
- Observed the following surfaces with debris build up; dry storage room shelves, handle and outside of the cookline microwaves, inside of white freezer, handles and inside of reach in cooler, and left side of fryers. Routinely clean these surfaces.
- 3-Compartment sink faucet is leaking. Contact a licensed plumber and repair fixture.
- Chlorine sanitizer in bucket was too high, >200 ppm. Manager emptied half the bucket and filled with fresh water, new concentration was still >200 ppm. Manager emptied more than half and added fresh water again; final concentration read 100 ppm. Maintain chlorine sanitizer in bucket at least twice the strength that is used for chlorine in the 3-compartment sink. Also sanitizer in 3-compartment sink was >200 ppm. After emptying half the sink and refilling, was still >200 ppm. Emptied and refilled again; final concentration was 100 ppm. Discussed with manager proper concentrations; 3 comp sink sanitizer should be less than sanitizer bucket since equipment/utensils are immersed in sanitizer for longer time.

- Observed a container of unlabeled Beef Stroganoff in the reach in cooler. The manager voluntarily threw the food out. All potentially hazardous food should be labeled and used/discarded in 7 days.
- Observed one thermometer at 38 F when calibrated. The thermometer was calibrated to 32 F during the inspection.
- The reach in salad cooler is missing a thermometer. Provide thermometers for all cooling units.
- Observed the shelves in the standing reach in cooler with rusty shelves. Repair. The fryer is missing its front cover. Replace.
- Observed the following non food contact surfaces with debris build up: handle and outside of the black microwave, two reach in stainless steal coolers in the kitchen, handle to the freezer in the kitchen, and both coolers in the dining area. Clean, and maintain routine cleaning.
- Observed the 3 compartment bar sink with a leak. Repair.
- Observed missing ceiling tiles in the chemical storage area. Replace.
- Observed stained ceiling tile in the kitchen. Clean/replace.

- Found small dish being used to scoop the cereal meal in a plastic storage container. Provide proper scoop.
- The following have shelves that are rusting: The storage rack above the 3-comp sink, rack shelving in the kitchen reach in coolers and bar reach in cooler. Repair/replace shelving. Observed tape being used to hold broken door seal gasket to the tall 2-door reach in cooler in the kitchen. Remove tape and replace seal gasket. Observed a few booth seats torn exposing padding. Repair.
- No bleach sanitizer test strips. Provide proper test strips if using bleach.
- Found heavy yeast/pink slime build-up inside the bar's ice machine. Also, observed food debris build-up on the counter mounted can opener in the back. Wash, rinse and sanitize.
- Wash, rinse and sanitize all the door seal gaskets to cooler/freezers, handles to coolers/freezers and the kitchen hand sink handles. Observed food debris build-up.
- Found plastic drinking cups in the bar stacked together still wet. Allow cups to be air dried.
- Floor/wall juncture tile in the kitchen in disrepair. Repair.
- Observed stained ceiling tile in the kitchen. Clean/replace.
- Several ceiling tiles in the kitchen and dry storage room in disrepair. Replace/repair.

- The racks in the reach in cooler in the prep area are rusting. Racks in the beverage cooler behind the bar are rusting.
- The rack in the pizza cooker have dried/baked on food debris. Wash, rinse, and sanitize this and all food contact surfaces.
- There is food build up in the handles of the coolers. There is build up on the door seal gaskets on all the coolers and freezers. Clean all non-food contact surfaces.
- Build up of dirt/dust on the floor especially under equipment in the dry storage area and under the 3-compartment sink. Clean the floors.
- There are several ceiling tiles throughout the kitchen and dry storage area that are bowing. Replace these ceiling tiles.
- The ceiling tiles and grid are stained. Clean the ceilings.
