The upright cooler with 4 doors by the salad assembly area is >41f. The thermometer on the outside reads 49f. Precooked egg patties are >50f. Upon seeing that the cooler wasn't working properly the manager and I had waited to see if the unit would go down in temperature due to being left open. After more than 10 minutes it had actually gone up a degree. Per temperature documentation the unit was working when the facility was opened. Per manager when he came in at 9am it was working properly. Everything in the cooler was moved to another cooler. The manager alerted maintenance to the issue. Do not use the cooler until it can maintain 41f or below. Please let me know when it is working properly. Also make sure temperature documentation is being done once it is in use.
Observed multiple employees not wash their hands properly. Many used their hands to touch the handle of the faucet to turn it off. You must conduct a training reinforcing proper hand washing. I need documentation of training and that all employees can wash their hands properly.
The hand sink by the salad assembly area is being used a dump sink. When I first went to the sink to wash my hands there was ice in the sink indicating a drink was dumped into it. Then an employee dumped a drink into it before washing her hands.
There is an employee wearing a bracelet who is doing food prep. Explained the employees in the front who do not do any food prep can have jewelry, but the ones doing food prep can't have bracelets.
There is buildup on the shelves in the back with the old microwaves and napkins. The cooler with lettuce needs cleaned on the bottom due to it appearing as if something has leaked. The coffee area needs cleaned. There is coffee and buildup throughout the area also on the cup dispensers.
There is a wrench on the faucet of the 3-compartment sink. Per manager there has already been a work order put in for it.
In the salad assembly area there was cardboard used on the floor. Spoke with employee and manager stating you can't use cardboard but can get a rubber mat. There are damaged floor tiles, especially by the mop area.
There is buildup on floors in the back area around the oil tanks.
There is a drip in the back by the cup storage. I pointed this out to the manager. Do not store anything that has been opened in the area.
There was freezer/cooler cleaner stored next to an opened box of tray liners. It was removed. There was also a spray bottle of glass cleaner close to a box of gloves. Discussed chemical storage.
There was a bottle of mucus cough relief medication on a shelf with test strips close to the hand sink by the salad assembly area. It was removed.

There are damaged door seal gaskets on many of the reach in coolers. Per manager they have ordered new ones. The milk cooler door behind the counter was open when I walked by it. Per manager their maintenance person will replace the hinge. Ensure that all employees know to check the door and make sure it is closed.
There is an accumulation of buildup in the holders for the coffee syrups and the containers holding sauces. There were employees assigned to cleaning these areas during inspection.
In the back of the facility by the water heater there is an accumulation of buildup that is on the floor and covering a pipe as well. An employee was assigned to clean it during the inspection.
There is a missing piece of coving by the walk in freezer.
There is paint coming off the walls in the back of the facility. I pointed this out to the manager.

Two employees were observed with no hair restrains. Ensure proper hair restraints are worn.
Found several plastic containers with cracks and chips. Replace containers used for food storage.
The following surfaces and items were found to have an accumulation of debris: Salad cooler near front counter, Ice cream machine and cooler near front counter, Smoothie station, Whip cream drawer at smoothie station, Orange Juice machine, Soda machine in drive thru, Milk cooler in drive thru, Soda machine in dining room. Wash, rinse, and sanitize these items and surfaces. Ensure routine cleaning.
