- Observed ham in cooler with a date label of 11/22, 8 days past first date of preparation. Manager voluntarily discarded ham. Ensure all food is used within 7 days of initial prep date.
- Observed debris buildup on front of flat top stove and fryers. Ensure routine cleaning of all non-food contact surfaces.
- Dumpster lid was open at time of inspection. Keep dumpster closed at all times.
- Observed duct tape-like material around ceiling vents. Remove any coverings that are not smooth, durable, nonabsorbent, and easily cleanable.
- Observed debris buildup on lower part of walls behind coffee machine, eye wash station and kitchen ice machine.

- Observed employee working with food and not wearing a hair restraint. Manager asked if employee had to wear one even though he is bald. I explained about bacteria occurring on the scalp and that a head covering acts as a barrier. Employee put on a hat.
- Found food debris inside ovens. Per manager, those ovens are not in use. Wash, rinse, sanitize, and maintain ovens until they are repaired and after.

- Lot of rust colored build up on the ice machine drip tray. Per manager, maintenance cleans the machine. Tell maintenance to include inside drip tray.
- Black build up on the walk in cooler/freezer door seals. Wash, rinse, and sanitize seals.
- Build up on floors especially under equipment. Clean the floors.
- Build up on walls especially near 3-compartment sink and dish machine. Clean the walls.

- There was wood under rack in dry storage as well as under the upright freezer in the prep area. They both appeared to have water damage, especially the one under the freezer.
- There were dead insects under the rack by the walk in cooler/freezer and in dry storage. The facility does have a licensed pest company that treats the facility (Harris Pest Control). Per invoice they were last out to the facility this week, but they did not have an invoice as they had not been billed yet. The last invoice in November indicated the pest operator had seen ants and was treating for them. I did not see ants what I had seen were more like a beetle. Discussed cleaning and possibly keeping one for the pest operator to see and treat for in the future. Once you get the new invoice please email to me. Also you need to clean and monitor for a week. If you see more you should make sure to tell your pest company. I need the latest invoice as well as an update as to whether you had seen anymore insects by 12-19-14.
