- Found unlabeled container of chopped ham in reach in cooler. Food was voluntarily discarded by manager. Ensure all food items are correctly date labeled.
- Observed an employee drink in walk in cooler near food items. Ensure all employee drinks are separate from food items and have a lid and straw to prevent contamination. Manager relocated employee drink on site.
- Observed debris buildup in the following areas: around grease trap, near drain board by 3-compartment sink, on pizza dough shelving, gaskets around walk in cooler, gaskets around upright white cooler, and gaskets around reach in prep coolers, as pointed out to manager.
- Observed a bucket in hand sink by 3-compartment sink. Ensure all hand sinks are easily accessible to all employees and used for hand washing only.
- Restroom door is not self closing. Ensure all restroom doors have a self closing latch.
- Observed debris buildup on floors around and underneath oven. Ensure floors are routinely cleaned.
- Observed debris buildup on walls throughout facility, specifically around front hand washing sink and oven. Ensure walls are routinely cleaned.

- Observed cups being used to dispense cheese on pizzas. Provide cups with a handle to prevent contamination of ingredients.
- No employees wearing hair restraints. Pulling hair into a ponytail or bun is not effective. Always wear a hat, hairnet, bandana, etc. when working with open food/prepping pizzas.
- The drying racks near the 3-compartment sink are chipping. Replace racks.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to seals, handles, racks, and sinks.
- Observed no self-closing devise on restroom door. Provide self-closing device to bathroom door and keep closed at all times except when necessary.
- Dumpster lid was open at time of inspection. Keep dumpster lid closed at all times to avoid attracting pests.
- Found build-up on floors especially under equipment. Clean the floors.
- Observed gap around plumbing into wall in employee restroom. Seal gap.
- Observed black build up and dust build up on walls especially behind 3-compartment sink and behind equipment. Clean the walls.

- The racks in the soda reach in cooler have peeling paint.
- The pizza oven racks have burnt on food debris. Clean the racks.
- The was food debris on all the counters in the kitchen area. Wash, rinse, and sanitize counters when finished working at them.
- All non-food contact surfaces need to be washed, rinsed, and sanitized. These surfaces include but are not limited to cooler door seal gaskets, walk in cooler handles, and reach in cooler handles and the bottom of the reach in cooler.
- Observed clean utensils stored in a dirty container near the pizza oven. Observed dirty sauce lids stored as clean.
- Dirt and dust build up on the floor especially under equipment, near the 3-compartment sink, and in corners. Clean the floors.
- Stains on the walls especially in the prep area. Black build up on the wall behind the 3-compartment sink. Build up on the ceiling in the walk in cooler. Clean the walls and ceilings.
- The 2 air vents above the pizza oven have excess dust build up. Clean the air vents.
