Observed gravy and sausage patties in the reach in cooler cooling down. Cool down documentation is not being kept for these items. When cool down occurs it should be documented. The manager and I discussed cool down methods. I will attach a cool down handout to the e-mail containing this inspection.

Observed leaky faucets at the 3-compartment sink and the prep sink by the stove on the back line. Repair or replace these faucets.
The wall under the dish machine drain board had a small hole in it and it was crumbling. Repair the wall. There was no caulk at the wall/dish machine drainboard juncture or the 3-compartment sink/wall juncture. Caulk these gaps to prevent water and debris from falling behind these gaps.

The shelves in the dry storage room had some build of food debris and a few mouse droppings were found on the plastic tray under the corn starch. Clean and sanitize these shelves and the outsides of food containers and canned goods.

Mouse dropping were observed on the floor along the base coving in the dry storage room (20+) and glue traps under equipment and shelves (5+ by each trap) in the kitchen, and a couple mouse dropping were found on the plastic tray under the corn starch on the dry storage shelf. Contact your pest control operator. Clean and sanitize all of the affected areas, outsides of canned goods and food containers. Discard any foods that may have become contaminated with rodent dropping. Contact Kim at TCHD when corrections have been completed for a re-inspection 309-925-5511 Ext. 301. Check the facility for and seal any openings which may allow mice to enter the building.
Replace the very rusty floor in the walk in cooler. Per manager, this is scheduled to be completed this summer 2015.

The wiping pail in the lunch room was at 100 ppm QUAT. Maintain at 150-400 ppm. Corrected to 200 ppm.

The walk-in cooler floor is very rusty. Repair or replace the floor.
Found chlorine sanitizer spray bottles at >200 ppm. The bottle in the cafeteria had so much chlorine that it bleached out the dye in the test strip. Maintain at 100-200 ppm. Corrected--The bottle in the dish room was labeled for use on the floors only and the bottle in the lunch room was diluted to 150 ppm.

Several of the chairs to the lunch tables have duct tape on them.
The floor to the walk in cooler is rusted and no longer easily cleanable.
