- Observed a single service dressing container being used as a scoop in the taco meat in the pizza make table. Provided a scoop with a handle.
- Repaint the rusty bottom of the wait staff salad/beer cooler so it is easily cleanable.
- Shelves through out kitchen had a build up of flour and debris. The inside of the pizza prep cooler, freezers, and can good rack also had a build up of debris. Wash, rinse, and sanitize these areas.
- The outside of the pizza oven and dough mixer kitchen had a build up of flour and debris. Wash, rinse, and sanitize them.
- Found build-up of debris on floors. Clean the floors, especially around the fryers. Remove oil dry from the floor..
- Repair/seal the hole in the wall by the hand sink by door to kitchen and replace missing base coving and repair water damaged wall in the mop room. Repair hole in the wall behind the fryers.

- Found a plastic cup being used as a scoop in the diced tomatoes in the waitstaff cooler. Provide a scoop with a handle.
- Repaint the chipping dough machines and rusty areas inside salad cooler.
- The shelves throughout the facility, soda lines, cooler and freezer interiors, and clean dish shelves had a build up of debris. Clean and sanitize them.
- The outside of the ice machine, outside of the dough mixer, outside of the hot holding cabinet had a build up of debris. Clean and sanitize these areas.
- Found build-up of debris on floors. Clean the floors.
- Reattach the base coving that has peeled away from the wall under the dish machine. Repaint the chipped/damaged walls in the employee rest room and mop room.
- The wall behind the dish machine and the ceiling and ceiling vents had a build up of debris. Clean the walls and ceilings.
- Found an unlabeled chemical (blue) spray bottle on a shelf above the 3-compartment sink. Chemicals must be labeled with the contents. Corrected--it was labeled Windex.
- Found a wet mop sitting in the bottom of the mop sink. Hang mops up to dry.
- Found chili that had been cooked and cooled in the walk in cooler, labeled and at 39F. Cool down is not allowed at Category 2 facilities. Spoke to owner and he stated he will contact Karen Irons 309-925-5511 Ext 288 regarding upgrading to category 1 facility.
