- Observed container of pepperoni labeled 12/20/2015 and container of salami labeled 1/1/2016 in walk in cooler. Today's inspection date is 1/19/2016. Manager voluntarily discarded foods. Ensure all potentially hazardous foods are correctly date labeled and stored for no longer than 7 days.
- Observed dust buildup on metal shelves for soda storage. Clean shelves on a routine basis.
- Observed food buildup on floors of walk in freezer. Routinely clean floors.
- Observed missing light shield in food walk in freezer. Install light shield. Manager ordered one on site.
- Observed fused light bulb in walk in cooler. Manager replaced bulb during inspection. Ensure all light bulbs are working.

- Observed the following: Rusty shelves in doughnut display unit, broken door seals in beverage coolers, and duct tape on ice machine door. Spray or paint rusty shelves, replace broken cooler door seals, remove duct tape on ice machine door and repair door.
- Observed build-up of dirt on doughnut display tong holder under the doughnut display unit. Clean. There is build-up of debris in the following areas: the spigot area of the soda and coffee machine in the general display section, shelves in soda coolers, and straw holder under coffee machine. Clean these areas.
- Observed build-up of debris on door seals of soda coolers the deli cooler and freezer. Clean door seals. Also observed dust on shelves, and debris in drawer used for coffee supply storage (below microwave oven) in the general display area. Clean shelves and drawer.
- The floor in the back storage section is dirty, especially in the area by the ice machine and hand sink. Clean floor.
- The ceiling in the walk-in soda cooler has build up of debris. Clean.
- Observed hole in ceiling above the soda lines on the back storage section. Repair.
- Sanitizer concentration in the kitchen 3 compartment sink was above 400 ppm. Maintain 200-400 ppm.

- Observed plastic food containers stacked together still wet stored as clean. Allow containers to be air dried.
- Observed gaps along the floor\wall juncture in the kitchen where some remodeling took place. Facility must seal gaps.

- Observed two bags of sausage laying on the floor in the walk-in freezer in the kitchen. Also, observed ice cream candy on the floor in the walk-in freezer. Place six inches off floor. Corrected.
- Observed cook washing hands then improperly turning hand sink valve handles off, re-contaminating hands. Turn valves off with a paper towel after washing hands.
- No hand drying towels were provided upon inspector washing hands in kitchen. Corrected.

- Back table near hand sink in kitchen has wood showing along the edges and corner. Per manager the table has been reported to corporate for fixing. Frayed door seal on walk in cooler.
- Build-up on handle of microwave under coffee machine.
- Build-up on ceiling tile frames in kitchen.
- Mop stored in water in bucket. Discussed with manager and employee proper mop storage.
