Observed new employee not wearing effective hair restraints.
The flour container does not appear to be food grade. The spatulas in the hot holding sauce area are melted on the handle and no longer easily cleanable.
The racks in the reach in cooler are rusty. The table in the wait staff area is not easily cleanable due to the coating coming off. There is cardboard under the microwave.
The equipment on the grill line has an accumulation of grease and food buildup.
The rack that holds plates has an accumulation of debris. The cabinet under the soda fountain has black buildup and is sticky. There is food debris in the bottom and the tracks of the Pepsi cooler. There is buildup in handles to reach in coolers/freezers and the outside of the microwave.
There is carpeting under the reach in cooler that holds salads.
There is coving that is missing and coming away from the wall in the kitchen by the dish machine.
There are damaged floor tiles in the area next to the carpeted area.
There is an accumulation of grease and food buildup around equipment in the kitchen.
There is food splatter on the wall by the prep cooler and grease accumulation by the fryers.

The facility didn't have any temperature documentation. The owner is aware he needs to complete it. You must submit 2 weeks of temperature documentation to me.

Found no chlorine sanitizer in dish machine. We ran it several times and both tried testing it with our test strips. I tried testing several different places in the dish machine as well. Maintain at 50-100 ppm. Per owner someone had been here 2 days ago to work on it and said it was working properly. At time of that person came back to work on it. After him turning up the sanitizer it is now 50ppm.

In the reach in cooler on the carpet some of the salads aren't covered as well as the containers for croutons.
There was a cup in the salt that didn't have a handle.
The grill, hood and fryer had an accumulation of grease and dust. Per owner the hood is behind on cleaning due to it being so cold. When they pressure wash it, it freezes.
There is carpeting under the reach in cooler that holds salads.
There is buildup on walls behind the dishmachine.
The wall by the counter oven is stained. Also the wall in the woman's restrooms is damaged.
The facility only has 1 food manager's license posted. Per owner there are 3, possibly 4, people with their license. You need to have all copies at the facility to verify there is someone present with their food manager's license at all times. I also emailed the owner a copy of the schedule for food manager's classes.
