- 229.167 (p) (11) (C)insects/pests not minimized. Evidence of live and dead roaches throughout kitchen. Bottom sink cabinets are covered in roach droppings. Observed several baby roaches at handsink and observed a live roach fall from cabinet onto foil covered plate of food after only a few minnutes of investigation.
- 229.167(p)(11)(C)rodents. Evidence of rodent droppings at back mopsink area behind water heater. Clean and remove all droppings from area within 24 hours.
- #22- One person per shift needs food manager certification on premise.
- 229.165 (f) (12) (B)integral thermomter. Need thermometers at all freezers and coolers containing potentially hazardous foods.

- Evidence of insects observed. Eliminate the presence of insects by using any approved means. INSPECTOR FOUND LIVE AND DEAD ROACHES AS WELL AS GNATS IN KITCHEN AREA. MANAGMENT STATES THEY DO THEIR OWN PEST CONTROL. CORRECT AS SOON AS POSSIBLE.
- No soap at the hand washing sink. Supply every handwashing sink with soap. SOAP AT HANDSINK MUST BE SPECIFIC TO HANDSOAP, DISHWASHING SOAP IS NOT ACCEPTABLE.
- ONE PERSON DURING ALL HOURS OF BUSINESS MUST HAVE A FOOD MANAGER'S CERTIFICATION TO PREPARE AND SERVE MEALS. NO ONE ON STAFF HAS ONE AT THIS TIME.
