Note: This was intended to be a change of ownership which unfortunately did not have all the new paper work requirements completed as the application was submitted 6/12/13 for take over 6/17/13 which was postponed to 6/21/13 which was then postponed to today's date. This inspection will serve as a final inspection, however, an additional inspection may be required if, once discussed with management, proper information as required by the new updated requirements/application are not provided to this office within ten business days. Provide the updated menu and drawing within ten business days as it was noted by the new owner that it would be moving shelving and cash register around.
HACCP: Discussed cleaning and sanitizing all cooking areas prior to opening the cooking area to ensure that times are clean and that a menu is provided to this office.
There has not been any 'interruption' in operations/sales. Machines and equipment are functioning and maintaining temperature.
Provide a copy of the Pest Control Operators last visit to ensure appropriate pest control is being maintained.
Frozen foods are frozen.
- Non-food contact surfaces designed, constructed, maintained, installed, located.
1) Various shelving units in the back storage area are rusted or chipping. 2) Cardboard boxes are being cut up and utilized as 'separators' in the retail space. 1) Paint or seal the shelving to ensure it is non absorbent, smooth and easily cleanable. 2) Provide plastic or other type material dividers to ensure they can be washed, rinsed and sanitized. Multi-use equipment and utensils shall be made and repaired with safe, non-toxic materials, including finishing materials that is corrosion resistant, non-absorbent, smooth, easily cleanable, and durable under conditions of normal use. Non-food contact surfaces such as splash areas or areas which otherwise require frequent cleaning, shall be designed and fabricated so as to be smooth, washable, free of unnecessary ledges, projections, or crevices, and to be readily accessible for cleaning, and to be of such material and in such repair as shall be easily maintained in a clean and sanitary condition.
- Thermometers, gauges, test kits provided
There were no test strips present for testing of the sanitizer. Provide test strips. These can be purchased at GFS or Pasquel's or the Peoria City/County Health Department or other provider of sanitizing equipment. A test kit or other device that accurately measures the parts per million concentration of the sanitizing solution shall be provided and used.
- Food contact surfaces of equipment and utensils clean
The interior of the ice cream freezer is soiled. The interior shelving of the walk in cooler is soiled. The interior of the Coca Cola cooler is soiled. The True freezer is soiled. Clean and sanitize routinely. The food-contact surfaces of all equipment shall be kept free of encrusted grease deposits and other accumulated soil.
- Non-food contact surfaces clean
Shelving in the dry storage rooms are soiled. Clean all areas routinely. Non-food contact surfaces of equipment shall be cleaned as often as is necessary to keep the equipment free of accumulation of dust, dirt, food particles, and other debris.

The floors behind the meat counter are soiled. The floors in the dry storage area are soiled. The floors in the walk in cooler are soiled. The floors under shelving and coolers are soiled. Clean all floors routinely. Floors, floor coverings, mats, and duckboard shall be maintained clean.
- Walls, ceilings, and attached equipment, properly constructed, clean
1) There is an uncompleted wall repair for plumbing behind the water heater. 2) Wall vents, walls in the walk in cooler and walls in the dry storage area are soiled. Repair the wall or provide paneling to minimize areas where pests may reside. Clean all walls routinely. Walls, ceilings, and attached equipment and decorative material shall be maintained clean. Walls and ceilings, including doors, windows, and similar enclosures shall be maintained in good repair.
- Premises maintained fee of litter, unnecessary articles. Cleaning and maintaining equipment properly stored, kitchen restricted to authorized personnel
1) The exterior bushes and fence line and behind the building has litter and debris. 2) There are various excess articles being stored in the back storage area. Remove all debris and maintain clean. Ensure all items are properly stored and clean all shelving before restocking back area. The premises shall be maintained in such condition as to prevent the harborage or feeding of insects or rodents. Only articles necessary for the operation and maintenance of the food service establishment shall be stored on the premises.
- No Certified Manager Present
The certified person provided was not 'certified' by the state of Illinois, only by the City of Chicago. Provided the reciprocity form for appropriate certificate reciprocity to be obtained. Original certificates of certified managers shall be maintained at the place of business and shall be made available for inspection.
