3-501.16(A)(2) Maintain PHF at 41 F or below. PHF in cold holding unit > 41 F due to faulty equipment. Cease use of equipment until repaired and verified by JCDH. RI cooler where sandwiches are stored 45째F. Sandwiches inside of RI cooler 45째F.
4-601.11 Wash, rinse, and sanitize fountain drink nozzles as often as necessary to keep free of slime mold and residue. Accumulation of residue on fountain drink nozzles.
6-501.111 Provide effective insect control. Observed roaches crawling inside of facility during inspection.
3-601.11 Cease the sale of food not labeled for individual retail sale as specified in law. Individual fruit cups iniside of the RI cooler. The following shelf stable items were found to be past the manufacturer date limitation: Borden Eagle Brand sweetened condensed milk expired on 0615/11.

Accumulation of caked on residue on nozzles of Red Diamond cappuccino machine.
6-101.11 Provide smooth, durable, nonabsorbent, easily cleanable floors, walls, and ceilings. Wall deteriorating near door where drink fountain in stored. 6-202.16 Provide perimeter walls and roof that protect establishment from weather, insects, rodents, and other animals. Ceiling leaking near storage closet. 6-501.12 Clean facilities (floors, walls, ceilings, fixtures, etc.) as often as necessary to maintain cleanliness. Accummulation of residue underneath shelving inside of WI cooler.

The following shelf stable products were found to be past the manufacturers date limitation: Campbells Cream of Celery soup epired on 10/22/11, Campbells Classic Tomato soup expired 11/18/11, and Pregresso light Beef Pot Roast soup expired on 08/31/11. Items were removed from the shelves and packed up to returned to vendor in order to receive a credit for items.
