No matter what religion or no religion...what do you think about a person that seems a pillar of humanity:
Gives to charities, tithes, volunteers at soup kitchens, smiles, opens doors, attends church,
mows the old lady's lawn.
BUT: In his mind...he thinks how stupid people are, lazy, good for nothings, hates the Arabs, Jews, Asians, blacks, homeless, Christians, too -
even women, kids, pets, liberals and conservatives -hates it all, wishes most ill...in his thoughts criticizes everything.
He just wants to appear as a 'good person'. Why?...mostly for his business reputation.
I would just say it sounds like a broken man somebody broke as a child but who knows, I cant figure it out, hate is the most self destructive force on the planet.

It's not unheard of.
One of my favorite lines from a parody political ad is where the candidate mentions that part of what he does in his ads is "to be seen talking to elderly people as if I didn't find them loathsome and irrelevant".
That encapsulates the sociopath's approach to life. They are usually capable of being charming, but it's all in the service of their career, whatever it is. They lack empathy but have learned to mimic it based on people's responses over time.

IMO,
If he is in a business and provides services for a fee, and the clients are happy then that's one part - and it's all OK.
If he has a mental condition that compels him to do so then I guess that's also fine. Can't blame him.
From the Islamic point of view, if he has bad or evil thoughts and he even has bad intentions BUT he has not taken any bad ACTION - then no harm done. It's OK.
However, if he has taken a BAD ACTION then there could be consequences in the hereafter (If God exists and if there is a judgement day, that is)
On the other hand, in the Islamic faith - if someone has good intentions to do something good but he she/was not able to take action for a genuine reason, then he/she still has her/her rewards saved with his Lord.
Having a clean heart with good intentions is the essence of Islamic faith. And it WILL be exposed on the day of resurrection.
Quran talks about it
Does he not know that when the contents of graves burst forth,
and the secrets of the hearts will be laid bare
Indeed, their Lord is All-Aware of them on that Day.

How would one knows such a person exists, or that a person you know is really like this inside, unless they tell you?
I read/listen to a lot of true crime. One of the podcasters always says yiu do not know another person's true self, ever, even if you are married to them or have lived with them a very long time.
Dennis Rader, Israel Keyes, the Craigslist Killer guy whose name escapes me. All people who appeared to be one way to the people who knew them but had something very different going on inside. Rader, the "BTK Killer" was only caught after a computer disk sent to police with a message taunting them was traced to the church where he was the business administrator. Churchgoer, family man, hung children from the rafters in their basement and laughed while they were dying, tortured other victims.
