The licensee shall inform the public in writing of the Chicago Department of Public Health?s Consumer Advisory relating to consuming raw and undercooked animal products. THE LARGE FOOD CONTAINERS HOLDING THE OATMEAL, SALT AND SUGER NOT LABELED. INSTRUCTED TO LABEL SAME.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. THE GREASE TRAP IN RUSTY LOCATED UNDER THE 3 COMPARTMENT SINK. INSTRUCTED TO CLEAN AND PAINT SAME.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DEFROST AND CLEAN ALL THREE REACH IN CHEST FREEZERS THICK WITH ICE BUILDUP LOCATED IN THE BASEMENT. ALSO CLEAN ALL THE REACH IN COOLERS HAS SPLASHED ON FOOD DEBRIS IN THEM.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN FLOORS UNDER THE COOKING EQUIPMENT 1ST FLOOR AND UNDER THE HEAVY EQUIPMENT IN THE BASEMENT HAS DUST AND DIRT BUILDUP ON THEM.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. REPLACE THE HOT WATER HANDLES BOTH EXPOSED HAND SINKS AND UTILITY SINK HANDLES ARE IN POOR REPAIR. REPLACE THEM. THE FAUCET AT THE 3 COMPARTMENT IN POOR REPAIR DRIPPING HOT WATER WHEN TURNER OFF. REPLACE IT. THE PIPE UNDERNEATH BOTH EXPOSED HAND SINKS DRIPPING WATER INTO BUCKETS. REPLACE PIPES. ROD OUT THE GREASE TRAP HAS GREASE BUILDUP.

FOUND THE 3 COMP SINK NOT MAINTAINED I.E.GREASE TRAP BOX BENEATH SAID SINK IS NOT CLEAN,SLOW DRAINING.MUST REPAIR AS REQUIRED.SERIOUS VIOLATION (7-38-030).
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE FOLLOWING REQUIRES A DETAIL CLEANING SHELVINGS AT COOKLINE,INTERIOR/EXTERIOR OF COOLERS,BOTTOMS OF FOOD PREP TABLES.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.FLOORS BENEATH THE COOKING EQUIPMENTS,FLOORS IN BASEMENT DRY STORAGE ARE DIRTY,CLEAN FLOORS IN DETAIL.

MUST INSTALL MISSING HANDLE/KNOB FOR HOT WATER AT THE MOP SINK AND VENTILATION COVERS IN TOILET ROOMS REQUIRE CLEANING.
