The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST SEAL RAW WOODEN FLOOR IN WALK IN COOLER. FLOOR UNDER DISH MACHINE MUST CLEAN AND KEEP DRY. MUST NOT USE CARDBOARD AS MATTING FOR FLOORS.
The walls and ceilings shall be in good repair and easily cleaned. WALL AT EXPOSED HAND SINK IN KITCHEN AND AT DISH AREA IN POOR REPAIR WITH HOLES/OPEN CREVICES, MUST SEAL. ALL HOLES IN CEILING THROUGHOUT BASEMENT MUST SEAL.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. MUST CLEAN AND MAINTAIN EXTERIOR OF GREASE TRAP.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. MUST INVERT STORED CLEAN POTS.
All food not stored in the original container shall be stored in properly labeled containers. ALL PREPARED BULK SPICES MUST BE LABLED.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. LIGHTS ABOVE DISH AREA MUST BE SHIELDED.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. ONE COMPARTMENT PREP SINK CLOGGED, MUST REPAIR TO PROPERLY DRAIN.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. ALL UNNECESSARY ARTICLES STORED THROUGHOUT BASEMENT MUST BE REMOVED OR STORED ELEVATED AND ORGANIZED.

All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED EVIDENCE OF RODENTS AND LIVE INSECTS ON THE PREMISES. OBSERVED 3 LIVE ORIENTAL ROACHES IN THE BASEMENT STORAGE ROOM. OBSERVED APPROXIMATELY 25 RAT DROPPINGS IN THE PREP AREA AND BASEMENT. MUST CLEAN AND SANITIZE THE AFFECTED AREAS. MUST ELIMINATE OR MINIMIZE PEST ACTIVITY. ADDITIONAL PEST CONTROL SERVICE IS NEEDED. SERIOUS VIOLATION 7-38-020.

DETAIL CLEAN THE EXTERIOR OF THE DISHMACHINE INCLUDING ALL SURROUNDING PIPES WHICH ARE ENCRUSTED WITH GREASE.

REPLACE ANY MISSING BASEBOARD THROUGHOUT THE PREP AREA. SEAL ALL WALL OPENINGS IN THE BASEMENT AND PREP AREA. DETAIL CLEAN THE WALL BEHIND THE COOKING EQUIPMENT.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. MUST PROVIDE A SELF-CLOSING DOOR DEVICE FOR THE BASEMENT WASHROOM.
