Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. GREASE TRAP EXTERIOR UNCLEAN AT PANTRY AREA UNDER 1-COMPARTMENT SINK. MUST CLEAN AND MAINTAIN EXTERIOR OF GREASE TRAP.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. OBSERVED FOOD BUILD UP INSIDE PREP AREA REACH IN COOLERS DOOR RUBBER GASKETS. MUST CLEAN AND MAINTAIN GASKETS.
The walls and ceilings shall be in good repair and easily cleaned. OBSERVED FOOD DEBRIS ON CEILING TILES ABOVE LOWER LEVEL PREP AREA. MUST CLEAN/REPLACE ALL UNCLEAN CEILING TILES.

REPAIR THE WATER LEAK BELOW THE MIDDLE COMPARTMENT OF THE 1ST FLOOR DISHROOM 3 COMPARTMENT SINK. REPAIR THE LEVER FOR THE RIGHT SIDE COMPARTMENT OF THE 3 COMPARTMENT SINK SO THAT THE SINK CAN FILL WITH WATER.
The walls and ceilings shall be in good repair and easily cleaned. DETAIL CLEAN THE DUST OFF OF THE CEILING TILES AROUND THE VENTILATION HOOD OF THE 1ST FLOOR KITCHEN. REPLACE THE CEILING TILES IN THE BASEMENT AND 2ND FLOOR PREP AREAS WITH CHIPPED PAINT. REGROUT THE BASEBOARD WALL TILES BEHIND THE BAR. REPAIR THE CORNER OF THE FRP BOARD IN THE 2ND FLOOR PREP AREA. REPAIR THE OPENING IN THE DRYWALL OF THE EMPLOYEE MEN'S RESTROOM IN THE BASEMENT.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. REPLACE THE CRACKED LIGHT SHIELD IN THE BASEMENT PREP AREA.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. MUST PROVIDE VENTILATION IN THE EMPLOYEE RESTROOMS IN THE BASEMENT.
