All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE FOLLOWING NOT CLEAN- OBSERVED FOOD DEBRIS/DUST BUILD UP ON SHELVES, INTERIOR OF CABINETS AT PREP AREA AND AT FRONT COUNTER, LIME BUILD UP INSIDE BAR GUN HOLDERS AND SLIGHT LIME BUILD UP INSIDE ICE MACHINE. MUST CLEAN AND MAINTAIN ALL UNCLEAN EQUIPMENT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED ACCUMULATED FOOD/DIRT BUILD UP INSIDE PREP AREA FLOOR DRAINS AND SPILLAGE ON FLOOR UNDER BAR COUNTER. MUST DETAIL CLEAN FLOOR AND INTERIOR OF FLOOR DRAINS.
All employees shall be required to use effective hair restraints to confine hair. MUST PROVIDE HAIR RESTRAINTS FOR ALL FOOD HANDLERS.
THE FAUCET AT THE 3-COMP. BAR SINK MUST EXTEND TO ALL 3-COMPARTMENTS. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.

OBSERVED 10-SMALL/FRUIT FLIES ON WALL & PICTURE AT THE DINING AREA OF THE 3rd FLOOR 'FRESH MARKET' CAFE; 6-HOVERING AROUND A BAG OF DIRTY LINENS BY REAR DOOR OF THE SAME CAFE; 5-ON BROOMS STORED IN A BUCKET BESIDE THE LINEN BAG. INSTRUCTED TO ELIMINATE SOURCE & CLEAN ALL AFFECTED AREAS. All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.
