All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. Found the ambient air temp. of walk-in cooler, at 59f. Unit tagged until fixed. Must fax letter to (312)746-4240 to untag unit. All cold holding units must have an air temp. of 40f or below.
All cold food shall be stored at a temperature of 40F or less. Found following food in walk-in cooler: 56-lbs. of eggs at 54-60f, 38-lbs of milk chocolate at 50-55f, 56-lbs of sliced tomatoes at 53-57f. Instructed to properly dispose of said products. (Total amt.=$545.11)
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Found 18 small/fruit flies at walls around the mop sink, about 20 hovering above a small bucket of dirty rags to the left of mop sink, 8 on walls of small employee room. Must eliminate source, clean affected areas & contact P.C.O.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Must keep dishroom floors dry at all times.

THE F.R.P. TO THE RIGHT OF THE WATER TANK IS PULLING AWAY FROM THE WALL, MUST FIX; SEAL ANY GAPS ALONG THE WALL & TILED BASEBOARDS BEHIND THE ICE MACHINE.
