All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. Found the ambient air temp. of walk-in cooler, at 59f. Unit tagged until fixed. Must fax letter to (312)746-4240 to untag unit. All cold holding units must have an air temp. of 40f or below.
All cold food shall be stored at a temperature of 40F or less. Found following food in walk-in cooler: 56-lbs. of eggs at 54-60f, 38-lbs of milk chocolate at 50-55f, 56-lbs of sliced tomatoes at 53-57f. Instructed to properly dispose of said products. (Total amt.=$545.11)
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Found 18 small/fruit flies at walls around the mop sink, about 20 hovering above a small bucket of dirty rags to the left of mop sink, 8 on walls of small employee room. Must eliminate source, clean affected areas & contact P.C.O.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Must keep dishroom floors dry at all times.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must fix leak at a pump in rear of storage room to prevent pest harborage.

Corrected at this time. Floor fans have been installed.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must fix a leak at the pump in rear of storage room, to prevent pest breeding/harborage.

OBSERVED OVER 20 LIVE SMALL FLIES ON WALLS/CEILING BY MOP SINK AND OUTSIDE OF WALK IN COOLER. MUST REMOVE INSECT ACTIVITY BY DATE OF REINSPECTION (09/16/11).
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. OBSERVED SLIGHT LIME BUILD UP INSIDE ICE MACHINE. MUST CLEAN AND MAINTAIN INTERIOR OF ICE MACHINE.
The walls and ceilings shall be in good repair and easily cleaned. MUST CLEAN ALL UNCLEAN WALLS AND CEILING AROUND MOP SINK AND REAR STORAGE AREA.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. PREP AREA HANDWASH BOWL HOT WATER HANDLE LOOSE(BY OFFICE) AND 3-COMPARTMENT SINK RIGHT SIDE COMPARTMENT STOPPER NOT WORKING. MUST REPAIR AND MAINTAIN SINKS.

INTERIOR PANEL OF ICE MACHINE & CUP HOLDER UNDER SODA DISPENSERS MUST BE KEPT CLEAN.
VERY POOR FLOOR GROUTING AROUND THE SODA DISPENSER, AROUND FRAPPE MACHINE & THE CRACKED/BROKEN CEMENT AT THE REAR HALLWAY/DRY STORAGE AREA-MUST FIX. FLOORS BEHIND ICE MACHINE & WATER TANK NEED DETAILED CLEANING (DEBRIS/UNNECESSARY ARTICLES FOUND).
THE F.R.P. TO THE RIGHT OF THE WATER TANK IS PULLING AWAY FROM THE WALL, MUST FIX; SEAL ANY GAPS ALONG THE WALL & TILED BASEBOARDS BEHIND THE ICE MACHINE.
