The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors around grease trap, behind shelves in walk-in cooler need cleaning, due to dirt/food debris.
The walls and ceilings shall be in good repair and easily cleaned. Filters & any attachments to interior of hood, with heavy grease build-up, must be removed & kept clean.
All food not stored in the original container shall be stored in properly labeled containers. All foods stored in walk-in cooler must be covered, dated/labeled.
All employees who handle food shall wash their hands as often as necessary to maintain a high degree of personal cleanliness and should conform to hygienic practices prescribed by the Board of Health. Observed foodhandlers in rear cooking area, moving a garbage can, or putting dirty pots/utensils - then going back to prepping food, without washing hands.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 3-COMPARTMENT SINK FAUCET LEAKING AT BASE. MUST REPAIR AND MAINTAIN SINK.

The walls and ceilings shall be in good repair and easily cleaned. OBSERVED FOOD DEBRIS ON WALL BEHIND PREP AREA HANDWASH BOWL. MUST CLEAN ALL UNCLEAN WALLS AND MAINTAIN.

ANY GROCERY BAGS USED FOR STORING FOOD IN THE FREEZER MUST BE REPLACED WITH FOOD GRADE BAGS. THE DOOR GASKET OF THE WALK-IN COOLER IS IN POOR REPAIR, MUST REPLACE.
FLOOR DRAINS UNDER THE 3-COMP SINK & AREAS AROUND THE GREASE TRAP MUST BE KEPT CLEAN.
CORRECTED
MISSING CAULKING ON THE SPLASHGUARD OF THE 3-COMP SINK, BETWEEN THE TILES & STAINLESS STEEL, MUST BE REPLACED.
ANY PLASTIC THAT IS WRAPPED AROUND THE BOTTOM PIPE(RIGHT SIDE) OF THE 3-COMP SINK, MUST BE REMOVED & FIXED.
