OBSERVED 10-15 RAT DROPPINGS ON FLOOR IN 2ND FLOOR STORAGE (WEST), 3 MICE DROPPINGS INSIDE OUTLET UNIT ALONG EAST STAIRWAY, 8 MICE DROPPINGS ON ELECTRICAL BOX IN 2ND FLOOR STORAGE (EAST). INSTRUCTED MANAGER TO CALL AN EXTERMINATOR FOR SERVICE, CLEAN AND SANITIZE ALL AREAS. FRONT DOOR NOT RODENT PROOFED 1/4' GAP ON BOTTOM OF RIGHT DOOR SEAL ALL OUTER OPENINGS ON DOOR TO PREVENT PEST ENTRY.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. SEAL ALL OPENINGS IN FLOOR AND REPLACE DAMAGED FLOOR TILES IN CLASSROOMS (FRONT, REAR, NORTH, ETC).
The walls and ceilings shall be in good repair and easily cleaned. REMOVE PEELING PAINT ON WALLS IN ALL AREAS (JAVA COFFEE SHOP PLAY AREA, STAIRWAY, ETC. SEAL BRICK WALLS IN CLASSROOM AND STORAGE AREAS. SEAL OPENINGS ALONG SOUTH WALL INSIDE CABINET IN LIGHTHOUSE ROOM. SEAL OPENING ALONG UTILITY SINK AND REPLACE MISSING CEILING TILE IN UTILITY CLOSET. SEAL LOOSE BASEBOARDS ALONG NORTH WALL IN HOSPITAL PLAY AREA. REPLACE MISSING BASEBOARDS IN ALL AREAS. SEAL OPENINGS UNDER EXPOSED SINK IN KITCHEN. SEAL OPENING AROUND LIGHT SWITCH IN 2ND FLOOR STORAGE (EAST). SEAL ALL OPENINGS, CRACKS, CREVICES, IN WALLS AND CEILINGS, WHERE WALLS AND CEILINGS MEET, AROUND PIPE FITTINGS, ELECTRICAL OUTLETS, ETC IN ALL AREAS (2ND FLOOR STORAGE, HALLWAY, CLASSROOMS ETC).
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR LEAKING FAUCET ON 3-COMP SINK.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. REMOVE UNUSED EQUIPMENT IN OFFICE AND STORAGE AREAS TO PREVENT PEST HARBORAGE. STORE MOP HEADS UPRIGHT TO PREVENT INSECT BREEDING.

Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.CLEAN DUSTY LIGHT IN FRONT OF HOOD IN KITCHEN PREP AREA. ONE LIGHT OUT IN CHILDERN'S RESTROOM NEED TO REPLACE.
The walls and ceilings shall be in good repair and easily cleaned.IN CLASSROOM 105 OBSERVED WATER DAMAGED CEILING TILES INSTRUCTED TO REPLACE.
