All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Must recaulk 3-compartment sink and handwashbowl (skewer area)to walls/seal all cracks/crevices.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Must detail clean floor under and around all shelf racks and along walls/corners at liquor storage room also must clean interior of all unclean floor drains.
The walls and ceilings shall be in good repair and easily cleaned. Must seal all small holes in wall by prep area 1-compartment sink and clean/sanitize walls adjacent to rear glassmachine.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 3-compartment sink pipe leaking under middle compartment and bar handwash bowl pipe leaking. Must repair and maintain plumbing.

Both 2-compartment sinks pipe leaking. Must repair and maintain plumbing.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Bar reach in coolers door rubber gaskets in poor repair, ripped. Must repair or replace all damaged coolers door gaskets.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Observed accumilated food/dirt build up inside prep and dishwash area floor drains. Must clean and maintain interior of all unclean floor drains.
The walls and ceilings shall be in good repair and easily cleaned. Observed small holes in wall behind 2-compartment sink by pot wash 3-compartment sink. Must seal all holes in walls.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REGROUT THE FLOOR TILES IN THE PREP AREA AROUND THE COOKING EQUIPMENT WHERE NEEDED.

CRACKED/BROKEN LIGHT SHIELD IN BUTCHER WALK-IN COOLER MUST BE REPLACED.
MISSING CEILING TILE ABOVE A PREP TABLE IN BUTCHER WALK-IN COOLER MUST BE REPLACED. BATHROOM VENTS NEED CLEANING ALSO.
ALL PREPARED FOOD IN COOLERS MUST BE DATED & LABELED.
CRACKED TILES & POOR FLOOR GROUTING AT THE BAR, DISHMACHINE AREA, & AT THE 3-COMP SINK ROOM MUST BE FIXED, TO PREVENT PEST HARBORAGE. STANDING WATER BEHIND THE ICE MACHINES MUST BE ADDRESSED.
FOLLOWING PLUMBING ISSUES MUST BE ADDRESSED: LEAK UNDER THE RIGHT COMP OF THE 3-COMP SINK, UNDER THE BAR DUMP SINK (BESIDE THE ICE BIN), UNDER THE SCRAPER SINK OF THE DISHMACHINE. THE STOPPER AT THE 'SANITIZING' SECTION OF THE 3-COMP SINK IS LOOSE, MUST FIX.
