A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. Found previous minor violation not corrected from report # 159411 dated 6-16-09. Violation 38. Found no back flow prevention devices for ice machine, coffee machine, and soda machine.serious violation. citation issued.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Found floors not clean at main kitchen areas along walls/under equipment.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. Found containers on shelving units not stored properly. (must store inverted)

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE FOLLOWING NOT CLEAN- OBSERVED ACCUMULATED FOOD BUILD UP ON EXTERIOR OF PREP COOLERS, SHELVES THROUGHOUT PREP AREA, PREP COOLERS DOOR RUBBER GASKETS WITH FOOD BUILD UP, FOOD ENCRUSTED ON LOWER SIDE OF DISHMACHINE DRAINBOARDS. MUST CLEAN AND MAINTAIN ALL UNCLEAN EQUIPMENT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED ACCUMULATED FOOD BUILD UP ON FLOOR UNDER AND AROUND COOKING AND COOLING EQUIPMENT AND ALONG WALLS/CORNERS. MUST DETAIL CLEAN FLOOR AND MAINTAIN.

MUST REPLACE ALL BURNED OUT BULBS AND PROVIDE/INSTALL CEILING LIGHT SHIELD COVERS/END CAPS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. PREP 3-COMPARTMENT SINK PIPE LEAKING UNDER LEFT SIDE COMPARTMENT. MUST REPAIR AND MAINTAIN SINK.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. REACH IN PREP COOLERS THERMOMETERS MISSING. MUST PROVIDE THERMOMETERS FOR ALL COOLERS.

MUST DETAIL CLEAN AND MAINTAIN THE FOLLOWING: INTERIOR OF THE DEEP FRYERS, STOVE, NOZZLES ON THE SODA MACHINE (IN THE WAIT STATION) AND THE PREP TABLES.
MUST DETAIL CLEAN AND MAINTAIN NTERIOR OF ALL FLOOR DRAINS AND FLOORS IN ALL THE CORNERS, UNDER/AROUND ALL HEAVY EQUIPMENT.
MUST REGROUT THE FLOORS AT THE ENTRANCE TO THE DISH WASHING AREA AND THROUGH OUT AS NEEDED.
MUST DETAIL CLEAN AND MAINTAIN WALLS AND CEILINGS IN PREP AREAS.
MUST REPLACE ALL MISSING/DAMAGED WALLS AND TILES IN PREP/STORAGE AREAS AND IN THE WAIT STATION.
MUST FIX THE LEAK AT THE PIPE UNDER THE 3 COMP SINK IN THE REAR MAIN PREP AREA. MUST FIX THE SLOW DRAINING EXPOSED HAND SINK BY THE 3 COMP SINK THAT IS ALSO IN NEED OF REPAIR IN THE REAR MAIN PREP AREA.
ALL COOKING EQUIPMENT MUST FIT UNDER THE VENTILATION HOOD IN THE MAIN PREP AREA-GRILL BY TALL REACH IN COOLER IS NOT COMPLETELY UNDER HOOD.
