Found floors not swept at rear of premises.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. Found cracked light shield at rear preparation areas.
Cleaning equipment shall be properly stored away from food, utensils, equipment, and clean linens. Found scattered mops and brooms throughout kitchen areas. (must store in one location)

MUST PROPERLY DATE/LABEL POTENTIALLY HAZARDOUS FOODS INSIDE WALK-IN COOLER THAT HAVE BEEN PREPARED/COOKED AND REMOVED FROM ITS ORIGINAL PACKAGING.
MUST RECAULK AROUND REAR EXPOSED HAND SINK AND WASH BOWL IN TOILET ROOM.
MUST DETAIL CLEAN AND MAINTAIN INTERIOR/EXTERIOR OF ALL COOKING EQUIPMENT.
MUST DETAIL CLEAN AND MAINTAIN FLOORS IN ALL CORNERS AND UNDER/AROUND ALL HEAVY EQUIPMENT.
MUST REPLACE BROKEN FLOOR TILE ON STAIR LEADING TO 2ND FL STORAGE.

MUST FIX THE LEAK AT THE BASE OF THE FAUCET ON THE MOP SINK.
REAR EXPOSED HAND WASHING SINK SLOWLY DRAINS. INSTRUCTED TO HAVE REPAIRED.
MUST RE-ROUTE WASTE PIPE UNDER BEVERAGE STATION TO A FLOOR DRAIN AND REMOVE CONTAINER USED TO CATCH WASTE WATER.
MUST STORE ALL UNUSED WASH CLOTHS IN A BUCKET OF SANITIZING SOLUTION.
ALL FOOD HANDLERS MUST WEAR A HAIR RESTRAINT.
