Violation descriptions and comments

Several opening noted on ceiling furnace room and side entry door. Instructed to seal opening on side of door and seal holes around pipes in furnace room.
Drain line from espresso machine and ice machine not properly installed. Instructed to install with drain line having air gap from floor drain.

Violation #35 now corrected.
Violation #38 now corrected.

Non food contact surfaces of ice machine, cooler shelving, racks not clean need cleaning.Bottom storage shelving in front counter areas need cleaning.
Found/observed rat droppings(25 ) in rear storage area in furnace room, behind furnace and hot water heater area in SPA area. Citation Issued#H000072730, with a court date of 10/06/2011, at 400 W Superior in room #112 at 2:00PM.
Floors under storage shelving, rear storage rooms, furnace room not clean need detailed cleaning(corners), floors behind ice machine, under heavy equipment, need detailed cleaning.
Damaged, stained, and missing ceiling tiles in storage room, and bathrooms, shall be replaced.
Exhaust vent(ventilation)in rear storage areas, and bathrooms need cleaning.
Stock items must be stred six inches off of floor to prevent pest harborage.
Missing light shields in rear storage room shall be provided.
