Floors along wallbase thru-out premises not cleaned. Instructed to detail clean daily.

Observed smoking in public places. Found customer-male standing at front counter smoking cigarette, while walking thru-out premises. Also, in video gaming area observed hundreds of cigarette butts and ashes on the ground around machines. Instructed that 'No Smoking' ordinances requires that establishment has 'smoking free inside all establishments and only allowed 15 ft. outside of establishment. Citation issued 7-32-015.
Handsink not provided in rear prep area where food preparation is performed. Instructed to install additional handsink in rear prep area for food handlers. Serious citation issued 7-38-030.
