- KDA_3_20215 - --On the customer shelf, there was one 16oz. can of La Preferida refried beans that had a severe dent around the seam. COS-can was removed from shelf.
- KDA_3_30511A1 - --In the back storage area, there were boxes of mustard bottles, bottles of shrimp sauce stored on the floor, under the racks of other boxes of stored food. COS-these boxes were moved to another shelf.
- KDA_3_50116A2 - --In the deli case, there was Greek pasta salad with cooked rice and spinach that was at 44.5F. Employee said it was made at 6:00AM. COS-left in deli case to cool down to 41F.
- KDA_3_50118A1 - --In the deli case, the following items were opened and held over 7 days. Chopped ham with a date mark of 9/7, oven roasted turkey with a date mark of 9/6, pickled loaf with a date mark of 9/7 and chicken breast with a date mark of 9/7/. COS-these items were voluntarily discarded.
- KDA_4_50111A - --The deli walk in freezer, has an accumulation of ice in all four corners. Also an accumulation of ice on the floor and some on the ceiling.
- KDA_4_50111B - --The seal on the bakery walk in freezer is torn.
- KDA_4_60111A - --In the deli ware washing area, there was one knife stored as cleaned in a knife strip that had dried on food debris. Stored on a shelf as clean were three metal pans with stickers on the bottom. COS- these items were placed at three compartment sink for rewashing.
- KDA_4_60111C - --The fan covers in the following locations have an accumulation of dust: deli walk in, both produce walk in's.
- KDA_4_90311B - --Stored as clean on a shelf near the deli three compartment sink, were several metal pans stacked wet. COS-pans were placed at three compartment sink for rewashing.
- KDA_5_20511B - --There was a yellow hose with a spray nozzle attached to it, sitting in the hand washing sink in the produce area. COS-hose was removed.
- KDA_5_20515B - --The hand washing sink in the bakery area, has a leak from the bottom of the unit when it is turned on.
- KDA_6_30112 - --At the deli hand washing sink, there were no hand drying towels. COS-paper towel machine was filled.
- KDA_6_50111 - --In the bakery walk in cooler, there is cracked and broken tile around the baseboard area.
- KDA_6_50112A - --In the storage area in the back, there is some single serve items under the shelves. In the deli walk in, there were three packages of string cheese and one block of cheese on the floor.
- KDA_7_20111A - --In the back storage area, there were three, 35lbs boxes of Crisco frying shortening next to Ecolab Chlorinated cleaner. COS-Shortening was moved away from the chemicals.
- KDA_Defacto - --deli case, coleslaw 41.9F, honey ham 40.9F, cooked chicken breast 38.3F, Colby cheese 40.6F.
