COS supplied (3). KDA_6_50111 - --Over 10 water damaged and missing ceiling tiles in dry storage area. Border damaged where floor and wall meet under 3 vat sink in dry storage area. KDA_6_501111A - --10 fresh rodent droppings in floor in back storage area under unused shelving to left of mop sink. 2 fresh rodent droppings next to 2 fresh rodent bodies in back storage area by water heater. KDA_6_501112 - --Total of 4 dead rodents in back storage area on floor and on glue traps. No bait or snap traps seen by inspector. KDA_6_501114A - --Lots of boxes of equipment, food, empty boxes in dry storage area. KDA_6_50112A - --Dirt buildup on window sills in establishment and on floor around equipment by walls. Soda syrup on floor of dry storage area. KDA_7_20111A - --In back storage room, spray cans of various cleaners stored on shelving next to sleeves of single use cups. KDA_7_20111B - --In back storage room, bottle of glass cleaner, can of goof off cleaner, and can of deicer stored over two boxes containing single serve packets of ketchup and mustard. KDA_7_30111A - --Bottle of cement cleaner stored next to packages of ramen noodles stored for retail sale in back storage area. KDA_Defacto - --Handsink in toilet room at 107F --Milk in reach in cooler at 41F, sandwiches in reach in cooler at 41F --No handsink in back storage area where 3 vat sink is located, but a handsink in front consumer area is sufficient. --No sanitizer observed on premise.
