  • KDA_3_50116B - --No pasteurized eggs 58F were over stacked in a food storage container that was stored on counter next to the grill top. PIC stated the eggs had been out since 6:00 AM. COS, eggs placed in the walk in cooler.
  • KDA_6_50111 - --Grease build up under the cooking equipment, and dried food debris around the base of the walls in the food prep area. Old mouse dropping were beside a cabinet space in the food prep area.
  • KDA_6_501112 - --Dead roaches were on the floor of the basement area. The basement area is for food storage, and single-serve items. PIC provided pest control invoice with a service date of 9-7-15.
  • KDA_7_20612 - --A open box/tray of rodent bait was inside a crawl space in the reach in freezer of the food prep area. COS, person in charge removed the open bait container that was not into a covered/tamper proof bait station.
