Observation:The following food contact surfaces have food debris build up: 1) behind and under all equipment in the kitchen, 2) Doors of all reach-in coolers and handles in the kitchen have build-up.

Observation: There is not hot water at handsnik in employee restroom.

Observation 1) Cove base around dish machine is no longer easily cleanable (old caulk).This is a repeat violation.

Observation: Several equipment and machines that are nonfunctional, no longer used or unnecessary stored in the back.

Observation: There is no enough pressure of hot and cold water at hand sink in restroom.
- Mechanical warewashing machine temperature measuring devices, heaters and baskets, and automatic dispensers, and alarms
Observation: The dish machine only reached 116 degrees after 5 times run (on date plate written 125*F minimum). Manager called the service and they explained them how to adjust the hot temperature. Corrected on site.(126*F).
- Plumbing system maintained in good repair
Observation: 1) Handwashing sink is not operating in restroom. Manager said the pipe froze 2 days ago.They called for service. 2) Three compartment sinks have leaks on faucets.
- Fingernail maintenance, polish, and artificial fingernails
Observation: One employee who is working with preparing pizza has long nails.

- Drying mops
Observation: Mop sitting in mop sink. When not in use a mop must be stored in manner that will allow air drying and not soil equipment of walls. Use provided mop rack.
- Food storage - preventing contamination from the premises
Observation: Shelve under cut table is not 6" off the floor. All food and equipment must be a minimum of 6" off the floor. Raise shelf.
- Outer openings are protected
Observation: Light is shining through bottom of back door. All outer openings must be protected from the entry of bugs and rodents. Apply weather stripping.

Observation: Light bulb around dish area is not working. Light must be adequate enough to aid in cleaning. Replace bulbs and/or make repairs as needed.

Observation: Dish machine sanitizer bucket was empty. Chlorine sanitizer must be maintained between 50-100 ppm for proper sanitation. Bucket replaced during inspection. CORRECTED.

Observation Faucet in food prep area is loosely attached. Reattach to maintain plumbing in good repair.
- Light bulbs, protective shielding
Observation: Light shields in walk-in cooler are cracked. All lights exposed to food and food items must be shielded or shatter proof. Replace shields.
- Food storage containers identified with common name of food
Observation: Shakers on line are not labeled. When removed from original container foods that are not easily identified such as spices must be labeled with common name. Label shakers.
- Equipment location, installation, repair, and adjustment
Observation: 1) Excessive frost build-up on walk-in freezer. All equipment must be maintained in good repair. Ace Refrigeration arrived during inspection, make repairs as needed.
- Cleanability of floors, walls, and ceilings
Observation: 1) Cove base around dish machine is not longer easily cleanable. 2) The following areas must have old caulk removed and be recaulked around mop sink and hand sink in food prep area. All floor/wall and sink wall/junctures must be easily cleanable.
