Observation: 1) Microwave oven in the kitchen has food debris

Observation: Didn't provide sanitizer at time of inspection in three compartment sink. Must provide QA sanitizer during all business hours. Corrected on site .COS --Observation: Sanitizer concentration in one small backet was too low at (100ppm). Must maintain between 200-400 ppm for proper sanitizing. Corrected on site.COS

Observation: Sliding door/ wall behind the beverage and ice machine is damaged, chipping away, solid at the bottom and arround and not cleanable. Walls /doors and physical facility must be cover and be maintained in good repair. Cover the whole or replace all three doors.

- Handwashing signage
Observation: Numerous handwashing sinks do not have when food employees must wash their hands. ALL handwashing sinks used by food employees SHALL HAVE A SIGN POSTED WHEN TO WASH THEIR HANDS. The food establishment has some CASEY'S handwashing signs and may keep those up too but these signs are not stating all the information required for the food code. The handwashing sink signs will be mailed and SHALL BE POSTED WHEN RECEIVED.

Observation: Several food items are labeled during this inspection except the FOLLOWING vinegar, oil and oregano. ALL working containers taken from original packaging shall be identified with the common name of the food. CORRECTED ON SITE.
- Posting inspection reports
Observation: The current inspection was POSTED BUT covered partially by other paper work and also not very visible due to a rack of cups was in front of the display of the paper work. The cups were moved to another location so the display case for the licenses (FE/RS) and inspections can be viewed at eye level for consumers. CORRECTED ON SITE.

Observation: 1)The pastry case does not have the large sticker posted on the case stating the ingredients in the pastries/donuts. The sticker is available from Casey's Corporate. Manager stated this facility use to have the sticker but the pastry case was changed. Obtain ingredient labels for allergens and post on the pastry case.

Observation: A wet wiping cloth was sitting on a food contact surface and not stored in sanitizer. Employee educated and verbalized understanding. ALL working wiping cloths used on food contact surfaces shall be kept in sanitizer between uses. CORRECTED ON SITE.

Observation: The ice scoop was sitting on top of the ice machine and has a LARGE CRACK. This ice machine has a built in ice holder. SHALL use the holder provided or another container that can be washed, rinsed, and sanitized. REPLACE the ice scoop due to the LARGE CRACK.

Observation: A small amount of Parmesan cheese shaker was sitting at room temperature. The container states KEEP REFRIGERATED. The Parmesan cheese was discarded during this inspection. New container of cheese was poured into another shaker and placed in the reach in cooler.CORRECTED ON SITE.

Observation: The exit door does not shut all the way. SHALL have ALL exit doors self closing and tight fitting to prevent entrance of pests and rodents. Repair as needed.
