  • Basic - Equipment and utensils not rinsed between washing and sanitizing. Detergent being used in middle sink with no rinse sink set up.
  • Basic - Equipment in poor repair. Microwave oven not heating product to safe temperature. Manager tested by attempting to heat water, 60 seconds on high setting with no temperature change.
  • Basic - Wet mop not stored in a manner to allow the mop to dry. Mop stored in water filled bucket.
  • High Priority - Cooked meat/poultry cold held at greater than 41 degrees Fahrenheit. Teriyaki Chicken at 49°. Manager closed lid on front line to maintain cooling temperature.
  • High Priority - Potentially hazardous (time/temperature control for safety) food reheated in a microwave oven for hot holding not reaching 165 degrees Fahrenheit. Meatballs heated within last 10 min not reheated to 165°, currently between 114°-130°. Manager attempted to reheat in microwave. Microwave is not heating product past 133°. Product removed from menu until it can be safely heated.
  • Intermediate - Accumulation of black/green mold-like substance on/around soda dispensing nozzles. Soda machine in dining area.
  • Intermediate - Handwash sink used for purposes other than handwashing. Dirty knife sitting in hand wash basin.
  • Intermediate - Ice chute and ice bin on self-service drink machine soiled/build up of mold-like substance/slime.
  • Intermediate - Proof of required state approved employee training not available for some employees. Employee training maintained at different location for one employee.
