  • Critical - Establishment operating without a current Hotel and Restaurant license, verified with D4 office license is deliquent. Repeat Violation.
  • Food-contact surface not smooth and easily cleanable, pegboard made of pressed cardboard type material where cooking utensils are hanging is water damaged and falling apart at the bottom. Repeat Violation.
  • Critical - Observed buildup of slime on soda dispensing nozzles, at the customer self service area.
  • Critical - Observed food being cooled by nonapproved method, approximately 10 pounds of cooked pasta cooling in a covered plastic bus tub on the bottom shelf of the walk in cooler is at 75 degrees F after 4 hours. Recommended cooling future batches of cooked pasta in shallow layers,sheet pans ,upper shelves on the speed rack, towards the rear of the cooler, and uncovered.
  • Observed gaskets with slimy/mold-like build-up, pizza make unit.
  • Critical - Observed improper use of bowl/plastic food container or other container with no handle used to dispense ready-to-eat food, paper cup used to scoop powdered sugar from bulk bin.
  • Observed reach-in cooler gasket torn/in disrepair, pizza and salad make unit's. Repeat Violation.
  • Critical - Observed sanitizing solution exceeding the maximum concentration allowed, chlorine wiping cloth santizer tested at 200 plus ppm. Repeat Violation.
  • Critical - Observed unlabeled spray bottle containing a clear substance hanging on the chenical storage rack, manager stated it smelled like "lemon cleaner" when asked what was in the bottle. Repeat Violation.
  • Observed wall soiled with accumulated black debris in dishwashing area.
  • Critical - Person in charge failed to insure consumer notification on use of clean buffet plates. Observed customer return to buffet with soiled plate and dispense pizza onto plate. Customer was not instructed to use a clean plate until inspector advised manager of situation.
