- Food-contact surface not smooth and easily cleanable, pegboard made of pressed cardboard type material where cooking utensils are hanging is water damaged and falling apart at the bottom. Repeat Violation.
- Critical - Observed buildup of slime on soda dispensing nozzles, at the customer self service area.

- Critical - Observed unlabeled spray bottle containing a clear substance hanging on the chenical storage rack, manager stated it smelled like "lemon cleaner" when asked what was in the bottle. Repeat Violation.
- Observed wall soiled with accumulated black debris in dishwashing area.
- Critical - Person in charge failed to insure consumer notification on use of clean buffet plates. Observed customer return to buffet with soiled plate and dispense pizza onto plate. Customer was not instructed to use a clean plate until inspector advised manager of situation.
