Observed roach activity as evidenced by 100 live roaches found throughout establishment / 50 were observed underneath kitchen garbage CONTAINERS in the cart wheels located near prep areas / 30 were observed throughout establishment on WALLS behind WOK STATION between pipes / 10 were observed underneath lower level shelving of prep tables where food was being prepared / 5 were observed walking on surface of BULK CONTAINERS used to store SUGAR FOR TEA and CORN STARCH / 5 were observed inside a LARGE HOLE behind ICE MACHINE.
  • Observed HEAVY grease accumulated under cooking equipment / WOK STATION .
  • Observed hole in wall behind ICE MACHINE live roaches were found inside HOLE .
  • Carbon dioxide/helium tanks not adequately secured/ CHAIN AROUND THE TANKS but not secured properly ,they still can fall over.
